Objective:Pneumonia in premature children,for example.To observe and compare the effects of different preventive measures on skin laceration caused by superficial vein indwelling needle dressing among premature infants with pneumonia, discuss the best method and provide guidance to clinical care.Methods:1. Selection of research objects:According to the inclusion and exclusion criteria developed for the present study,90 prematures with pneumonia in a tertiary hospital in Changzhou City were selected from September 2015 to September 2016.2. Grouping and method:These patients were randomly divided into conventional nursing care group,3M protective film group and Sanyrene group,30 cases for each group. Conventional nursing care group was given conventional nursing care, that is,3M transparent dressing was directly used after the puncture of superficial vein indwelling needle; for 3M protective film group,3M painless protective film was sprayed before pasting 3M transparent dressing based on conventional nursing care; for Sanyrene group, Sanyrene was applied before pasting 3M transparent dressing based on conventional nursing care.3. Observation items and time:(1) Comparison on the baseline:the three groups were compared in terms of general data.(2) 4 days after place indwelling needles for prematures with pneumonia, conventional nursing care group,3M protective film group and Sanyrene group were compared in terms of frequency and occurrence rate of skin laceration, number of positive cases and positive detection rate of indwelling needle catheter bacterial culture, time required for dressing pasting after the puncture of superficial vein indwelling needle, neonatal skin condition score (NSCS), EDIN, neonatal infant pain scale (NIPS), satisfaction score of infant patient's relatives and additional medical cost.Results:1. Comparison of general data:There were no statistically significant differences in general data between conventional nursing care group,3M protective film group and Sanyrene group (P>0.05), suggesting that their general data were comparable.2. Frequency and occurrence rate of skin laceration:there were statistically significant difference between conventional nursing care group,3M protective film group and Sanyrene group (P<0.05). The frequency and occurrence rate of skin laceration of 3M protective film group were smaller than those of conventional nursing care group (P<0.01), while those of Sanyrene group were smaller than those of conventional nursing care group (P<0.05). There were no statistically significant difference between 3M protective film group and Sanyrene group.3. Number of positive cases and positive detection rate of indwelling needle catheter bacterial culture:There were statistically significant difference between conventional nursing care group,3M protective film group and Sanyrene group (P<0.05). The number of positive cases and positive detection rate of indwelling needle catheter bacterial culture of 3M protective film group were smaller than those of conventional nursing care group (P<0.01) and Sanyrene group (P<0.01), and those of Sanyrene group were smaller than those of conventional nursing care group (P<0.05).4. Time required for dressing pasting after the puncture of superficial vein indwelling needle:There were statistically significant difference between conventional nursing care group,3M protective film group and Sanyrene group (P<0.01). The time required for dressing pasting after the puncture of superficial vein indwelling needle of conventional nursing care group was smaller than those of 3M protective film group (P<0.01) and Sanyrene group (P<0.01), while that of 3M protective film group was smaller than Sanyrene group (P<0.01).5. NSCS:There were statistically significant difference between conventional nursing care group,3M protective film group arid Sanyrene group (P<0.01). The three dimensions (degree of drying, erythema and skin damage) of 3M protective film group were better than those of conventional nursing care group (all P<0.01) and those of Sanyrene group (all P<0.01), and those of Sanyrene group were better than those of conventional nursing care group (P<0.05 for degree of drying and skin damage; P<0.01 for erythema).6. EDIN:There were statistically significant difference between conventional nursing care group,3M protective film group and Sanyrene group (P<0.01). The five dimensions (facial movement, phsycial movement, sleeping quality, contact with parents or dependents and console) of 3M protective film group were better than those of conventional nursing care group (all P<0.01) and Sanyrene group (P<0.01 for facial movement, contact with parents or dependents and console; P<0.05 for phsycial movement and sleeping quality); those of Sanyrene group were better than those of conventional nursing care group (P<0.01 for phsycial movement, sleeping quality, contact with parents or dependents and console; P<0.05 for facial movement).7. NIPS:There were statistically significant difference between conventional nursing care group,3M protective film group and Sanyrene group (P<0.01). NIPS of 3M protective film group was better than those of conventional nursing care group (P<0.01) and Sanyrene group (P<0.01), while that of Sanyrene group was better than that of conventional nursing care group (P<0.05).8. Additional medical cost:There were no corresponding values since the conventional nursing care required no additional materials.3M protective film group and Sanyrene group requires very low additional medical costs. The additional medical cost of 3M protective film group was lower than that of Sanyrene group, showing a statistically significant difference (P<0.01).Conclusion:In order to prevent skin laceration caused by superficial vein indwelling needle dressing, the use of 3M painless protective film can reduce the occurrence rate of skin laceration as well as the risk of infection, save nurses'working time, relieve infant patients'pain and improve the satisfaction rate of relatives, without any obvious increase in medical cost.
