Research background Venous indwelling needle was originally used in the pediatric ward and it was born out of necessity in order to reduce the children’s fear of the repeated puncture and injection and relieve their parents’ anxiety and emotion. With the development of society and nursing, venous indwelling needle is widely applied. And from the initial application of the pediatric ward, it extends to almost all clinical departments, But as the initial model of indwelling needle application, such as a brief interruption of transfusion during ordinary infusion process of Ma Yueqin’s theory, with 2 ml heparin dilution of the scalp needle tube to a sterile dressing and scalp needle and syringe fixed properly at the same time, Meng Fanrong’s disposable vein indwelling scalp needle in CT inspection application value and so on, all of them prove that the scalp needle still have some value in clinical application. This paper through the discussion of the application effect of scalp indwelling needle in treatment of patients with special needs from neurology wards, thus to further broaden the train of thought which brings convenience to the patients, reduce the cost of medical care and save the health resources.Purpose: To discuss in practice, because of economic reasons or other reasons to reject the use of conventional superficial vein indwelling needle or it isn’t easy to indwelling venous indwelling needle because vascular damage seriously but we have to face several intermittent treatments every day. Under these conditions, to discuss the application effect of scalp needle indwelling among commonly used infusion methods.Methods: 1. Grouping method: Select three hundred patients from a department of neurology in Jiaozuo of Henan province from January to August in 2012. To discuss the clinical application effect, divide into three groups randomly and each group consists of one hundred patients: Superficial vein indwelling needle group; Scalp indwelling needle group; The conventional infusion group.2. Implementation method: Superficial vein indwelling needle group is the superficial vein indwelling needle puncture for the treatment of puncture needle, after the completion of each batch of treated tube sealing, leave for 3 days; Scalp acupuncture group is based on the conventional infusion scalp needle for puncture needle, the day of intermittent treatment is completed and seal directly the tube of scalp needle, after the intermittent treatment, then pull out the needle; Conventional infusion group received routine infusion puncture, puncture each time when each intermittent treatment is finished, then pull out the needle.3. Observation method: The three groups are in superficial vein indwelling needle of the shortest retention time of 3 days for an experimental observation period, and intermittent infusion two times at 5pm and 1pm at night every day as the standard.4. Outcome measures: Observe the consequences under the intermittent therapy within 3 days, 3 groups of patients because of indwelling needle body or infusion treatment which bring about the consequences of pain, sleep time, phlebitis, infusion costs, limb activity time, transfusion leakage, catheter blockage and air embolism etc.5. Statistical method: The laboratory data of three groups of patients are analyzed by using SPSS16.0 software, count data is received by using chi square test and measurement data is received by using analysis of variance and t(level of significance is d=0.05)test.Results: 1.Sleep Time: Superficial vein indwelling needle group and Scalp indwelling needle group have great differences between the conventional infusion group in sleep time. The average sleep time of superficial vein indwelling needle group is 6.22, scalp needle indwelling group is 7.29, the conventional infusion group is 7.56. The sleep time of superficial vein indwelling needle group is shortest, the sleep time of the conventional infusion group is longest(P<0.01).2.Pain Degree: Pain levels of three groups are mild pain, among them, the level of superficial vein indwelling needle group is grade 3 which accounts for the larger ratio, the pain lever of scalp needle indwelling group is grade 1 which pain ratio is bigger and the level of the conventional infusion group is grade 2 which accounts for the larger ratio(P=0.0083).3.The number of puncture: in the study of three days, puncture pain of each person in Superficial vein indwelling needle group is one time, puncture pain of each person in scalp indwelling needle group is three times and puncture pain of each person in the conventional infusion group is nine times.4. The cost of treatment: within three days, the cost of superficial vein indwelling needle group treatment is 215.27 yuan, the cost of Scalp indwelling needle group is125.27 yuan and the cost of the conventional infusion group is 47 yuan.5.Limb activity time: according to the intermittent treatment each day for three times, survey the completely independent activity time of puncturing the limb in without the needle state, the time of superficial vein group is 0 hour in 3 days, and the time of scalp needle indwelling group is 17.55 hours in 3 days, and the time of routine group is 55.5 hours in 3 days. The differences are statistically significant(t=82.86,p=0.00).6. Complications: there are no statistical significance in comparisons between these complications of the three groups(P>0.05), which indicates no significant differences between the three groups of transfusion methods in terms of complications.7. Vessel selection: because the needle of scalp needle indwelling group is tinier than that of Superficial vein indwelling needle group, so scalp indwelling needle group has more advantages in vascular choice.Conclusion: Scalp needle indwelling transfusion is another infusion method between needle infusion and conventional infusion transfusion between superficial vein indwelling. Comparing to superficial vein indwelling needle, the scalp indwelling needle has more advantages in many aspects: the times of puncture less; less pain; not depriving patients of normal sleep time and lower cost. And it has better clinical efficacy. What’s more, it can be used as one of the common clinical infusion methods for the special needs patients and a beneficial supplement of common clinical infusion method.
