Survey And Analysis On Self-management And Self-efficacy Of Patients With Coronary Heart Disease In Three Hospitals In Northeast China

Purpose:This study was to investigate the self-management behavior and self-efficacy of coronary heart disease(CHD) patients in northeastern China, and to explore the relationship between self-management behavior and self-efficacy in order to provide theoretical basis for intervention strategies.Method:This study used convenience sampling method, in April 2015 to 12 months in Shenyang, Changchun and Harbin and other places representative of the three top three hospital clinics, and more than ten years in the northeastern region of coronary heart disease patients in each hospital A total of 600 patients were enrolled in this study. Questionnaires were collected from the general questionnaire, the Self-management Behavior Scale for Coronary Heart Disease, and the Self-efficacy Questionnaire of Coronary Heart Disease(CHD). The data were collected and analyzed with SPSS19.0 statistical software.Result:1.The total score of self-management was(77.46 ± 13.71) in the three hospitals of northeastern China, and the score was 67%.(39.65 ± 9.06) points, score of 59%; emotional management dimension score(11.53 ± 2.41) points, the score of the score of the score of the medical management(39.65 ± 9.06) The target is 58 per cent. The self-efficacy score of CHD patients in three hospitals in northeast China was(32.91 ± 8.58) points and the score was 51%.(23.86 ± 6.35), 50% and(9.05 ± 2.84), respectively, and the score was 48%.2.The total score of self-management behavior and the dimensions of patients as the dependent variable, the single factor analysis of meaningful demographic data and disease characteristics as independent variables into the equation, the total score of self-management behavior of the multiple regression equation results show: The coefficient of determination R2 = 0.454, F = 40.69, P <0.01, the equation is statistically significant. In this multiple regression equation, the standard coefficient of per capita income, education level, co-resident, payment method of medical expenses, smoking history and other factors were positive, and the standard coefficient of occupation was negative.(P <0.05).3.The self-efficacy total score and the various dimensions as the dependent variable, the single factor analysis of meaningful demographic information and disease characteristics as independent variables into the equation, the total score of self-efficacy multiple regression equation: equation decision Coefficient R2 = 0.318, F = 22.78, P <0.01, the equation is statistically significant. In this multiple regression equation, the standard coefficient of influence factors such as duration, place of residence, cohabitation, drinking history and other factors were positive, while the standard coefficient of occupation, marital status and gender were negative, all statistically significant(P < 0.05).4.The Pearson correlation analysis showed that there was a positive correlation between total score of self-efficacy and total score of self-management, self-efficacy score of self-efficacy and self-management behavior score(r = 0.425, P <0.001). There was also a positive correlation between self-efficacy scores and self-behavior score(r = 0.254-0.440, all P <0.01).Conclusion:This study investigated the self-management behavior and self-efficacy of 600 patients with coronary heart disease who had been diagnosed and treated in the top three hospitals in northeastern China and lived in northeast China for more than ten years by convenient sampling method. The following conclusions were drawn:1. The level of self-management behavior of patients with coronary heart disease in the middle low level, self-efficacy level is low.2.self-management behavior of the total score of the factors include: occupation, educational level, with or without cohabitation, family income, medical expenses, smoking history.3.The influencing factors of total score of self- efficacy included gender, occupation, marital status, residence, co- inhabitants, course of illness, drinking history.4.The higher the level of self- efficacy, the higher the level of self- management behavior; the higher the level of self- management behavior, the higher the level of self- efficacy. Health care workers should start from the level of self-efficacy, ease the patient's negative emotions, improve patient compliance, universal health education, thereby enhancing self-efficacy and self-management level, improve coronary heart disease prognosis.
