Influence Of Risperidone,Quetiapine And Ziprasidone On Metabolic Indicators In Patients With First-Episode Schizophrenia

Abstract/Summary:
ObjectiveTo study the influence of three kinds of atypical antipsychotics (risperidone, quetiapine, ziprasidone) on metabolic indicators in the treatment of first-episode schizophrenic patients.Methods120 patients with first-episode schizophrenia were randomized into risperidone group, quetiapine group and ziprasidone group treated with risperidone, quetiapine and ziprasidone respectively for 12 weeks. All patients were assessed with Positive and Negative Syndrome Scale (PANSS) at baseline and at the end of the 4th,8th,12th week of the treatment to evaluate the severity of disease, and were measured to obtain the data of waist circumference, three acyl glycerin (TG), high-density lipoprotein cholesterol (HDL-C), fasting plasma glucose (FPG) at baseline and at the end of the 4th,8th,12th week to evaluate the prevalence of metabolic syndrome (MS).ResultsThere was no significant difference in response rate among the three groups (89.2% in risperidone group,88.9% in quetiapine group,86.5% in ziprasidone group; P>0.05). Waist circumference, TG and FPG in risperidone group and quetiapine group increased significantly compared with baseline from the end of the 4th week of the treatment. At the end of the 12-week treatment, waist circumference, TG and FPG in three groups all increased significantly compared with baseline (P<0.05 or 0.01), and level of HDL-C in risperidone group and quetiapine group decreased significantly compared with baseline (P<0.05). The prevalence of MS, waist circumference, level of TG in risperidone group and quetiapine group at each interview-point were significantly higher than those in ziprasidone group (P<0.05 or 0.01).ConclusionThe overall efficiency of risperidone, quetiapine and ziprasidone is equivalent for the treatment of first-episode schizophrenia, however, the influence on metabolic indicators is different among the three atipical antipsychotics. Influences on metabolic indicators of risperidone and quetiapine are greater than the effect of ziprasidone.
