Objectives To investigate of the relationship of obesity and metabolic syndrome(MS)with adiponectin(ADP),leptin(LEP)and cytokines in acute episode schizophrenia patients.Furthermore,the correlations of psychopathology with MS were analized.Methods A cross-sectional study was conducted in Hui-Long-Guan Hospital from December 2016 to June 2017.The levels of serum LEP,ADP,IL-1?,IL-6 and TNF-? were measured using an enzyme linked immunosorbent assay(ELISA)in all subjects.Compared the LEP,ADP and cytokine levels between the MS and non-MS groups;the relationship of psychopathological symptoms and MS was analyzed.Statistical analyses were all conducted using the SPSS 20.0 statistical package for Windows.P<0.05 was considered as statistically significant.Results Comparison of BMI,waist circumference,blood pressure,blood glucose,triglyceride,high density lipoprotein between MS group and non-MS group were statistically significant(P<0.001).There was no significant difference in the positive symptoms,negative symptoms,general psychopathological symptoms and PANSS total score in the two groups(P>0.05).The scores of PANSS anxiety(t=2.356,P<0.05)and tension(t=-3.783,P<0.001)in the MS group were higher than those in the non-MS group.Compared with the MS group,the levels of leptin(t=-2.656,P<0.01)and adiponectin(t=-4.316,P<0.001)were significantly different from those in the non-MS group.The total number of leukocyte in the two groups showed significant difference(t=-3.612,P<0.001).There was no significant difference in IL-1?,IL-6 and TNF-? between the two groups(P>0.05).Logistic multivariate regression analysis showed that the increased adiponectin was a protective factor for MS(OR=0.271,95%CI:0.115~0.638),and leukocyte increase was a risk factor for MS(OR=2.861,95%CI:1.188~6.833).The Partial correlation coefficients were separately conducted between BMI and LEP,ADP,WBC and inflammatory factors controlling for these covariates including gender,age,first or relapse and disease duration.The results showed that BMI was positively correlated with LEP(r=0.363,P<0.05),WC was positively correlated with WBC and IL-6(r=0.413,P<0.01;r=0.342,P<0.05).ADP was significantly correlated with BMI,WC,TG and HDL-C(P<0.05).The total score of PANSS and general psychopathological symptoms was positively correlated with TG(all P<0.05).Among 16 items of general psychopathological symptoms,no cooperation was negatively correlated with TG and action retardation was positively correlated with systolic blood pressure and diastolic blood pressure.In addition,there was a positive correlation between worry about physical health and WC,and abstract thinking was positively correlated with systolic blood pressure(all P<0.05).Conclusions Acute phase schizophrenia accompanied with MS have high incidence of obesity;Elevated adiponectin is a protective factor for MS,Leukocyte elevation is a risk factor for MS;MS patients' psychopathological symptoms such as anxiety and tension increased,non cooperation,action retardation,worry about health and abstract thinking are all related to MS.
