Objective: Through the retrospective analysis of 54 patients with stage ? / III colorectal cancer,the statistical differences were found between the age,sex,TCM syndromes,treatment and other aspects of postoperative liver metastases.To study the factors influencing the liver metastasis of colorectal cancer,to provide theoretical basis for the prevention and treatment of liver metastasis of colorectal cancer combined with traditional Chinese medicine and western medicine,especially the role of traditional Chinese medicine in prevention and treatment of liver metastasis,with the view to the future in the clinical rational use of integrated traditional Chinese and Western medicine treatment methods to delay or even prevent the liver metastasis of colorectal cancer purposes.Method: In this study,retrospective analysis was used.According to the relevant inclusion and exclusion criteria screening in January 2014-January 2017,54 patients with outpatient treatment of primary colorectal cancer(surgical staging ? ~ III)in Hubei Province Hospital,Department of oncology and after surgery,use chi-square test to analyze and summarize the relevant factors that may affect the liver metastasis of colorectal cancer.Result: 54 patients,including 34 patients with liver metastases,20 patients with non-liver metastases.24 men,accounting for 44.4%,30 women,accounting for 55.6%.(1)According to the age group divided into less than 60 years old group is greater than 60 years old group,of which less than 60 years old group of 29 people,more than 60 years old group of 25 people,there was significant difference between the two groups(P <0.05).(2)According to the primary tumor site is divided into colon and rectal group,the original in the colon of 29 people,primary in the rectum of 25 people,According to X2 test statistics,there was no significant difference between the two groups,colorectal cancer liver metastasis or tumor origin has nothing to do.According to colorectal cancer surgery with lymph node metastasis,lymph node metastasis group and lymph node metastasis group,lymph node metastasis of 39 patients(72.2%)was significantly greater than the proportion of patients without lymph node metastasis(27.8%),and with the liver metastasis between the statistical difference(P <0.05),indicating that surgery also found that lymph node metastasis is leading to colorectal cancer liver metastasis adverse factors.(3)The number of liver metastases in the low and undifferentiated adenocarcinomas was the highest among the four pathologic types,and the number of liver metastases was significantly higher than that of the primary tumor.Indicating that the organization of low and undifferentiated adenocarcinoma in patients with the most prone to liver metastasis.(4)Preoperative tumor marker levels were also associated with colorectal cancer liver metastases.Colorectal cancer-related tumor markers CEA,AFP and CA-199,CEA and CA-199 preoperative elevation may lead to colorectal cancer liver metastases.(5)The probability of liver metastases was the highest and the difference was statisticallysignificant(P <0.05).The difference between liver and spleen syndrome was also higher than that of liver and spleen(P <0.05);liver and kidney yin deficiency syndrome(11.8%),and liver metastasis of colorectal cancer is not significant(P>0.05),compared with other syndromes were statistically significant(P <0.05).(6)In patients with untreated or no postoperative adjuvant chemotherapy,the number of liver metastases in patients with high liver metastases(70.6%)and those who did not take or irregularly took Chinese medicine was significantly higher than that of non-liver metastases(P <0.05).There was a significant difference(P<0.05)between the patients who had taken traditional Chinese medicine for 1 year,and had a significant difference(P <0.05).(7)There was no significant correlation between the sex of the patients and the primary site of the tumor and the liver metastasis of colorectal cancer,and there was no statistically significant difference between the two groups according to the comparison between liver metastasis and non-liver metastasis(P<0.05).Logistic regression analysis was used to analyze the factors that were statistically significant in univariate analysis.The results showed that age,lymph node,chemotherapy completion,TCM medication time and preoperative CEA level were independent effects on liver metastasis of colorectal cancer(P<0.05).The OR values of age,lymph node and preoperative CEA were> 1,which indicated that these three indexes were the risk factors of liver metastasis leading to colorectal cancer.Chemotherapy and the use of traditional Chinese medicine time OR value <1,indicating that the completion of chemotherapy in patients with liver metastasis is lower.Conclusions: The results showed that age,tumor type,preoperative CEA and CA-199 levels,whether lymph node metastasis,chemotherapy completion,TCM syndromes and traditional Chinese medicine were analyzed by single factor and multivariate analysis of 10 factors in this paper Treatment time is associated with liver metastasis of colorectal cancer.Age is greater than 60 years old prone to liver metastasis;low and undifferentiated adenocarcinoma is the most pathogenic type of tumor that causes liver metastases;preoperative CEA and CA-199 increased easily lead to the occurrence of liver metastases;TCM syndromes in the spleen and kidney deficiency is most prone to liver metastasis,liver and spleen do not have evidence of liver metastasis may also lead to failure;Post-adjuvant chemotherapy or irregular administration of traditional Chinese medicine patients prone to liver metastasis,this shows that non-standard treatment and in place of treatment will increase the risk of recurrence and metastasis.According to the NCCN guidelines for the treatment of treatment and long-term traditional Chinese medicine conditioning can reduce the risk of liver metastasis of colorectal cancer is a favorable factor in the prevention and treatment of liver metastasis.Clinical attention to these types of risk factors,to be regularly monitored and effective prevention of treatment.For effective treatment programs should also actively persuade patients with treatment,in order to reduce the incidence of liver metastases in colorectal cancer.
