Study On The Levels Of Vitamin D,Cytokines,Blood Glucose And Hepatorenal Function In Diabetic Patients With Toxoplasma Gondii Infection

Objective: To investigate T.gondii infection in diabetic patients,and to explore the immune function of diabetic patients with Toxoplasma infection and its influence on blood glucose and hepatorenal function.Methods: 1.405 diabetic patients(DM group)and 150 normal controls(NC group)were selected from the Affiliated Hospital of Guizhou Medical University.According to the detection result of Toxoplasma antigen-antibody,DM group was divided into DM with Toxoplasma infection positive group(DM-TP group)and DM with Toxoplasma infection negative group(DM-TN group).2.Peripheral blood samples were collected from all subjects.The anti-T.gondii specific antibodies(IgG,IgM)and circulating antigen(CAg)were detected by enzyme linked immunosorbent assay(ELISA);529bp gene fragment of Toxoplasma gondii was amplified by polymerase chain reaction(PCR)to detect Toxoplasma DNA;The levels of IFN-?,IL-2 and IL-4 in plasma were detected by ELISA.Collected 25(OH)D3,fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c),liver function [total protein(TP),albumin(ALB),alanine acyl transferase(ALT),aspartate amino transferase(AST)] and renal function[serum creatinine(Scr),blood urea nitrogen(BUN),uric acid(UA)and urinary microalbumin/urinary creatinine(UmALB/Ucr)].3.The data were analyzed by SPSS 17.0 software: the count data were checked by chi-square test,the measurement data among groups were analyzed using ANOVA or rank test.Correlation analysis was performed by Pearson correlation analysis.The difference was statistically significant with P<0.05.Results: 1.The seroprevalence of T.gondii infection in diabetic group and control group was 19.75% and 6.67%,and the difference between the two groups were statistically significant(P<0.05).The positive rate of IgG antibodies,IgM antibodies anti-T.gondii and circulating antigen(CAg)in DM group were 17.28%,2.72% and 0.49%,there were three patients that IgG antibodies and IgM antibodies anti-T.gondii were doublely positive.It was not detected IgM antibodies to T.gondii and circulating antigens(CAg)in NC group.2.In this study,Toxoplasma gondii specific nucleic acid fragments were not detected in all the subjects,and therefore,it failed to carry out the genotyping of Toxoplasma.3.The level of 25(OH)D3 obviously decreased in DM-TP group and DM-TN group(16.75 ±4.69ng/ml,19.70 ± 4.41ng/ml)compare with NC group(37.55 ± 7.35ng/ml),and DM-TP group was lower than DM-TN group(P <0.05).3.The levels of IFN-?,IL-2 in DM-TP group(52.08 ± 9.25pg/ml,174.48 ± 68.69 pg/ml)was significantly higher than DM-TN group(45.07 ± 7.89pg/ml,137.37 ± 27.26 pg/ ml)and NC group(40.20 ± 7.61pg/ml,135.30 ± 26.82 pg / ml).The level of IL-4 in three groups had no statistical difference(P>0.05).5.The concentration of FBG and HbA1 c in DM-TP group,DM-TN group and NC group were(11.23 ± 2.27mmol/ L,9.32 ± 1.98%),(9.24 ± 2.74mmol/L,8.11 ± 1.27%),(5.01 ± 0.84mmol/L,4.78 ± 0.59%).Pairwise comparison between groups was statistically significant(P <0.05);The levels of TP and ALB in DM-TP group were lower than DM-TN group and NC group(P <0.05),while the level of ALT in DM-TP group was higher than DM-TN group and NC group(P <0.05);The levels of Scr and UmALB / Ucr in DM-TP group were higher than DM-TN group and NC group,Pairwise comparison between groups was statistically significant(P <0.05).But AST,BUN and UA in three groups were not statistically significant(P> 0.05).6.Correlation analysis: 25(OH)D3 had negative correlation with HbA1 c,IFN-? and IL-2(P<0.05),but no correlation with FBG and IL-4(P>0.05).Conclusion: 1.The infection rate of Toxoplasma in diabetic patients was higher than that in healthy subjects.2.Vitamin D deficiency and imbalance of Th1/Th2 maybe lead to immune function disorder in diabetic patients with T.gondii;3.Toxoplasma infection may have some effects on blood glucose and liver and kidney function in diabetic patients.4.The positive rate of Toxoplasma DNA in human peripheral blood was low.
