Application Of Doula-induced Labor Analgesia In Reducing Labor Pain In Primiparous Women

ObjectiveChildbirth is a natural process of human reproduction,which is caused by the contraction of the uterus and tension and fear of the psychological pain of childbirth.Most women,especially first-time mothers feel this process is very painful.The purpose of this study was through the application of GT-4A in Doula delivery analgesia apparatus,monitored and evaluated the analgesic effect of maternal labor process,effects of postpartum hemorrhage and neonatal asphyxia,so as to reduce maternal pain,improve maternal pain threshold,increase the natural birth rate,reduce the rate of cesarean section.Subjects and MethodsThis study is a quantitative research design,the use of sequential cluster sampling method.Selected 200 cases of primipara in a comprehensive 3A hospital in Shandong Province from January to December in 2016 as the main object of study.In this study,Randomly divided into two groups based on the basic principle of voluntariness,Selected the GT-4A analgesia instrument of Doula delivery analgesia parturient in the observation group,the same period before the implementation of labor analgesia maternal control group.100 people/group.The control group was followed the usual treatment.Treatment method of the observation group is the production process when the women entered the active phase,the palace mouth expansion to 3cm,or pregnant women need analgesia,the midwife give analgesia to the parturient.GT-4A Doula delivery analgesia apparatus has 4 output lines,A and B are connected to the two women around electric pole piecwithe,C,D of the two connections in the maternal lumbosacral,according to the clinical parameters,the midwife can adjust the current strength appropriately,adjust to the early maternal muscles can be slightly degree.When the maternal cervix was expanded to 10 cm,the midwife stoped using the guide music labor analgesia.In this study,IBM Statistics 19 for Windows was used for statistics analysis.Statistical methods included t test,single factor variance analysis,ca test and descriptive statistical analysis,and the standard deviation of the measurement data was indicated by the standard deviation.The main outcome measures were analgesic effect,length of labor,amniotic fluid,postpartum hemorrhage and neonatal effects.Results1.In this study,200 cases of primiparous women who were delivered in a comprehensive 3A hospital in Shandong Province from to December in 2016 were split into two groups according to the voluntary principle.Each group of 100 people voluntarily chose GT-4A guided analgesia Pain maternal delivery is the observation group,the same period did not exercise labor analgesia in the control group.The age and gestational age of the two groups were compared,There were no difference between the two basic profiles(P>0.05),grouping comparisons could be made2.Analgesic effect of analgesia group The analgesic effect of the observation group was apparently higher than that of the control group(P<0.05).The VAS scores of 30min,1h,2h,3h,4h and 5h were apparently lower than before(P<0.01).After analgesia,the 6hVAS score also decreased significantly(P<0.05).The mean onset time was 12.23±4.30min,the mean time of onset was 5.18±1.08,which was significantly lower than that of analgesia before the average score of 7.36±1.32(P<0.01).Due to the shortening of labor time,Active period basically did not reach 6-8h.3.The active time of the observation group was 134.40±54.20 min,the active time of the control group was 195 ±65.40 min.total labor of the observation group was 468 ±162 min,total labor of the control group was 534±27 min.the active period and total stage of labor were less than the control group.There was significant difference between the two groups(P<0.05).There was no significant difference between the second and second stage of labor.4.There were no significant differences in Apgar score and amniotic fluid traits between the observation group and the control group(P>0.05).There was no obvious adverse reaction in the fetus or neonatal group.5.The mean blood loss of the observation group was 123.75 ±42.45ml,the average blood loss of the control group was 165.60±40.15ml.The average bleeding rate in the observation group was 81.45±19.12ml and the average bleeding rate was 121.05 ± 19.27ml.ConclusionGT-4A Doula delivery analgesia apparatus is an effective method for analgesia.It is through the D-T wave to stimulate the hands of the hands of the tiger's neck and wrist region,through the nerve conduction,activation of their own analgesic system,improve maternal pain threshold,reduce pain.The GT-4A Doula delivery analgesia apparatus is safe and uninvasive,its onset time is very fast,no inhibitory effect on uterine sympathetic motor nerve and somatic motor nerve,does not affect the labor force,had no inhibitory effect on the uterus and pelvic floor muscle.lt can relieve the labor pain,fear,tension,solve the maternal pain syndrome,shorten the production process,the initiative to effectively mobilize the participation of maternal childbirth,it can satisfy the natural childbirth conditions were more willing to choose natural childbirrth,reduce the probability of mental factor of cesarean section?GT-4A Doula delivery analgesia apparatus can accelerate the production process,promote vaginal delivery,and noninvasive,convenient operation,no influence on mothers and infants,pregnant women and their families are easy to accept.Therefore,this study suggests that GT-4A is a safe and effective Doula delivery analgesia apparatus,simple operation,easy to use,not only can achieve good pain relief,and can effectively promote cervix uteri dilated,shorten the labor process,promote natural delivery,thereby reducing the rate of cesarean section,maternal and fetal adverse reaction and has high clinical value and social meaning considerable less advantages,and is suitable for China.It can be widely used.
