Objective:To study the feasibility and effect of enteral immune nutrition support inperioperative period for patients with gastric cancer.Methods:70cases of gastric cancer were randomly divided into2groups.35cases inthe experimental group (enteral nutrition group,EN group) were applied with EnteralNutritional Emulsion(TPF-T) as a kind of liquid diet in preoperative period from the7th day to the12th hour before the operation, and then12to24hours after operationwere supported with Nutrison Fibre in advance. The other35cases in the controlgroup(traditional therapy group) were made the preoperative preparation by thetraditional method which was implemented by the steps as follows, semi-liquid fooddiet from the7th day to the12th hour before the operation, and then fasting until theoperation, fasting-infusion-liquid-semifluid in postoperative period.Compare thedifferences between the above two groups in these cases: postoperative intestinalfunction recovery time, postoperative hospital stay,bloating, abdominal pain, nausea,vomiting, diarrhea and other adverse reactions, and the occurrence of postoperativecomplications after surgery. Parameters including serum albumin (ALB), transferrinprotein (TRF), prealbumin (PA), retinol binding protein (RBP), the total bilirubin(TBil),CD3+T cells and CD4+/CD8+ratio were measured and compared on the7thday before operation(preoperative period), the day of surgery(sugery day) and on the8th day after operation when the nutritional support ended(postoperative period).Results:.In the experimental group(EN group), postoperative intestinal functionrecovery time faster(P<0.05), adverse reactions and the occurrence of postoperativecomplications fewer(P<0.05), postoperative hospital stay shoter(P<0.05)than those inthe control group. ALB, TRF, PA, RBP, CD3+T cells and CD4+/CD8+ratio on the dayof operation were higher than the control group(P<0.05) and better than7th daybefore operation (P<0.05). On the8th postoperative day, ALB, TRF, PA, RBP, CD3+T cells and CD4+/CD8+ratio of the experimental group were higher than controlgroup,but had no significant rise compared with that of surgery day(P>0.05), yet ofthe control group, were all significantly lower(P<0.05). On the8th postoperative day,TBil of control group was higher than experimental group and obviously higherthan that of7th preoperative day and surgery day(P<0.05).Conclusion: Enteral immune nutrition(Nutrison Fibre) could be applied to thepreoperative bowel preparation with better effect and less adverse reaction.Postoperative early enteral nutritional could promote the recovery of gastrointestinalfunction, ameliorate nutritional status and cell immune function and reduce theoccurrence of adverse reaction and complication.
