Objective To investigate the cytokins IL-33,sST2,IL-4?IL-5 and IL-6 expression levels in peripheral blood of patients with Henoch-Schonlein purpura(HSP)in children and to explore their clinical significance so as to provide scientific evidence for targeted treatment.Method During the time between August 2015 and December 2015,collected the Peripheral Blood from 27 HSP patients and 22 healthy controls which were all from the Provincial Hospital affiliated to Shandong University.HSP group was divided into acute group and recovery group.Patients were accord with European League Against Rheumatism and Paediatric Rheumatology European Society(EULAR/PReS)criteria for the diagnosis of allergic purpura in 2006.Healthy controls had no history of recent infection,blood systems and other systems related diseases.The expressions of IL-33,sST2?IL-4?IL-5 and IL-6 were measured by enzyme linked immunosorbent assay(ELISA)and the mRNA expression of IL-33 and sST2 were studied by real-time quantitative polymerase chain reaction.Results:1.The serum value of IL-33 of HSP group in acute period(365.5160.6pg/ml)was higher in recovery period(186.7101.5pg/ml)and in normal group(175.992.8 pg/ml)(P<0.05),but there was no significant difference between the level of the IL-33 in recovery period and normal group(P>0.05).The serum sST2 level in children with acute period of HSP(1788.6523.8pg/ml)was higher in recovery period(1182.5455.7pg/ml)and in normal group(1083.6489.6pg/ml),but there was no significant difference(P>0.05).2.The serum IL-4 level in children with acute period of HSP(20.114.7pg/ml)was higher in recovery period(16.312.1pg/ml)and in normal group(16.913.0pg/ml)(P<0.05).The serum IL-5 level in children with acute period of HSP(18.73.9pg/ml)was higher in recovery period(9.72.9pg/ml)and in normal group(9.13.2pg/ml)(P<0.05).The serum value of IL-6 of HSP group in acute period(208.731.3pg/ml)was higher in recovery period(40.16.2pg/ml)and in normal group(32.44.7pg/ml)(P<0.05),but there was no significant difference between the level of the IL-33 in recovery period and normal group(P>0.05).3.The ratio of sST2/IL-33 in the acute stage(4.843.21)of HSP was much lower than those in the recovery stage(6.813.57)and in the normal controls(6.773.84)(P<0.05),but there was no significant difference between the ratio of sST2/IL-33 in recovery stage and normal group(P>0.05).4.Quantitative Real-time PCR revealed that IL-33 mRNA levels were up-regulated in acute period of HSP by 5.392.08-fold(P<0.05)and 5.47 1.97-fold(P<0.05)compared to recovery period of HSP and normal controls.IL-33 mRNA levels differences were no significant between in the recovery period of HSP and normal controls(P>0.05).sST2 mRNA levels were increased in acute stage of HSP by 2.971.91-fold(P<0.05)and 3.13 2.01-fold(P<0.05)compared with recovery period of HSP and normal controls.But there was no significant between in the recovery stage of HSP and normal controls(P>0.05).5.The ratio of sST2/IL-33 mRNA levels in the acute stage of HSP was significantly lower than those in the recovery stage and in the normal controls(P<0.05),but the ratio of sST2/IL-33 mRNA levels between recovery stage of HSP and normal controls were no significant difference(P>0.05).Conclusion:1.The serum value of IL-33?IL-6?IL-5 and IL-4 of HSP group in acute period was higher in recovery period and in normal group,suggesting the four are closely related to the development of HSP.2.sST2 is a decoy receptor that sequesters IL-33 and blocks the IL-33/ST2L signal pathway.Moreover,the serum sST2 level in children with acute period of HSP was higher in recovery period and in normal group.It shows that sST2 may be a protective effect for HSP.3.The ratio of sST2/IL-33 in the acute stage of HSP was much lower than those in the recovery stage and in the normal controls which suggested that the imbalance of sST2/IL-33 might be important factors resulting in HSP and restoring the balance of sST2/IL-33 may be a new tactic in treating HSP.
