Evaluation Of Short-term Clinical Efficacy On Treating Oral Lichen Planus (Pattern Of Spleen Deficiency And Dampness Accumulation) By Traditional Chinese Medicine Decoction Assisted With Intradermal Needles

Objective:In this research, we'd like to observe the short-term clinical efficacy of treating oral lichen planus with both Chinese medicine decoction and intradermal needles, then we could evaluate if the intradermal needles would enhance the effect of herbal decoction, so that we can figure out the possibility of using herbs combined with needles in the treatment of oral lichen planus, which could not only widen the application of intradermal needles but also provide another way of thinking in the Traditional Chinese Medicine treatment for oral lichen planus.Method:60 outpatients suffering from oral lichen planus from Department of Stomatology in Sichuan Provincial Hospital of Traditional Chinese Medicine were allocated into group A (use both intradermal needles and herbs decoction) and group B (use herbs decoction only),30 subjects in each group. All subjects matched with the standards of OLP diagnosis and spleen deficiency and dampness accumulation pattern. The course of treatment was 30 days in total. Comparing the curative effect (including sign scores, VAS, dry, numb and rough sensation in the mouth) after treatment in different stages (day7, day 14, day30) and also the total effective rate between these two groups. All statistics were dealt with software SPSS 19.0.Results:1. the total efficiency:After treatment, in the treatment group, the total efficiency is 93.33%; in the control group, the total efficiency of 83.33%, the difference was not statistically significant (P> 0.05).2. analysis of signs scoring:After the treatment, lesions condition of both groups were improved. On D7, D14, D30, the signs scores in group A were no better than that in group B, and the difference was not statistically significant (P> 0.05); on D7, D14, D30, the improvement of signs scores in group A were better than that in group B, the overall decline of signs scores in group A in larger than that in group B, the difference was not statistically significant (P> 0.05). Meanwhile, the decreased proportion of signs score in group A in larger than that in group B.3. analysis of VAS score:After the treatment, both groups have pain relief. On D7, D14, D30, the VAS scores in group A were no better than that in group B, and the difference was not statistically significant (P> 0.05); On D7, the relief degree of VAS in group A is slightly better than that in group B, however, on D14 and D30, the results were reversed. The overall decline of VAS scores in group A in lower than that in group B, the difference was not statistically significant (P> 0.05). Nevertheless, the decreased proportion of signs score in group A in larger than that in group B.4. analysis of local symptoms:After treatment, some patients with dry and sticky sensation in the mouth, numb sensation, rough sensation have relieved, the treatment group was better at releasing rough sensation in the month and this symptom relieved better than the others from the overall perspective of view.Conclusion:1. Chinese medicine decoction Jian Pi Hua Shi Fang has an positive effect on treating OLP (pattern of spleen deficiency and dampness accumulation), from the local point of view, it could relieve mucosal edema and congestion,, reduce pain or other discomfort, relieve dry and sticky mouth, mucosal numbness, rough sensation and other symptoms, and overall, it helps reinforce the spleen and remove the dampness in order to enhance the healthy qi and reduce the evil and enhancing physical constitution.2. In this study, the intradermal needles treatment did not show significant assistant effect, which not only because the experimental design itself is not rigorous, but also may be related to the current intradermal needles treatment for OLP is lack of methodology and evaluation criteria and other relevant reference. Therefore, future research directions should be treated as the methodology of intradermal needles treatment for OLP in order to establish a systematic approach and standard evaluation system, and to provide reference for future research.
