Osteoarthritis (OA) is one of the common diseases in orthopaedics. OA can be caused by many factors, OA began in cartilage wear, secondary synovial hyperplasia, sclerosis of subchondral bone and the degeneration of joint capsule and surrounding muscles. Clinical manifestations of limited for knee joint movement, knee joint pain and knee joint deformation, clinical medication can delay the occurrence of osteoarthritis, and to improve clinical symptoms. Yunnan baiyao tincture has certain treatment function to osteoarthritis, Yunnan baiyao tincture with external use is slowly absorbed, curative effect is relatively poor. The previously study showed that Yunnan baiyao by intra-articular injection has a role in osteoarthritis. In the study, we use Yunnan baiyao by external use and intra-articular injection in treatments of knee osteoarthritis in Sprague-Dawley (SD) rat. Then, we contrast that the effects of two methods for the treatment of osteoarthritis.We choosed that 40 healthy SD rats and were ramdomly divided into fourth groups:Normal (A group), Control (B group), External use (C group), intra-articular injection (D group), with 10 rats in each group in our study. The anterior cruciate ligament in B, C and D groups were cut bilaterally. The rats in B group was no drug use (once,0.1 ml/d in each knee joint). Yunnan Baiyao was external used to bilaternal knee joint in C group (twice,0.1 ml/d in each knee joint). Yunnan Baiyao was injected into knee articular cavity in D group (once,0.1 ml salin/d in each knee joint). The treatment of drug was began at 5 day after surgery. Two rats in each group were killed on each of the following:3 d,7 d,2 weeks,4 weeks, and 8 weeks after surgery. Four samples of knee joint were collected for observation of the changes in the expressions of CGRP and ED-1 and morphology of knee joint. To explore the treatment effects of external use and intra-articular injection of Yunnan Baiyao in SD rats with knee osteoarthritis by assessing the expression of calcitonin gene related peptide (CGRP) and the monoclonal antibody ectodysplasin-1 (ED1), and the changes in the shape of the knee joint. Moreover, we contrast that the effects of two methods for the treatment of osteoarthritis.According to the results of our research, we found the conclusions as below:1. Morphology observation, compared with group A, the sclerosis of subchondral bone of the group B and C as well as D, there are more damage of articular cartilage and synovial hyperplasia than group A. Among the groups, group B is the heaviest destruction of articular cartilage and synovial hyperplasia. Group C and D are obviously lighter than group B. At 8 weeks, the knee joint femoral condyle cartilage Mankin's score from low to high:group A, group D, group C and group B. After statistics processing, there was statistical significance between two groups, while the ACL preserved in group A and the cartilage best preserved;the group BCD of Clipping ACL of cartilage damage is heavier. Cartilage damage in group D was relatively lighter than Group C and D.2. Compared with A group, the expression of CGRP in B, C and D groups are rised by different extent. The expression of CGRP in B group is significant increased than D group. There is a significant difference between C and D groups (P<0.01). The synovial hyperplasia is not severe in the ACL preserved,which the CGRP fiber is hard to find, and in the group BCD of clipping the ACL the synovial tissue hyperplasia of CGRP are more serere, which the CGRP of group D of the hyperplasia the mildest. Compared with group C and D, the expression of CGRP in group B had a significant difference (P<0.01). Compared with group C, the expression of CGRP in group D is obviously milder (P<0.01).3. Compared with A group, the expression of ED-1 in B, C and D groups are rised by different extent. The expression of ED-1 in B group is significant increased than D group. There is a significant difference between C and D groups (P<0.01). The synovial hyperplasia is not severe in the ACL preserved,which the ED-1 fiber is hard to find, and the group BCD of clipping the ACL synovial tissue hyperplasia of ED-1 are more severe, which the ED-1 of group D of the hyperplasia the mildest. Compared with group C and D, the expression of ED1 in group B had a significant difference (P <0.01). Compared with group C, the expression of ED-1 in group D is obviously milder (P<0.01).4. In groups C and D, macroscopic view showed the cartilage wear in group C is more severe than group D. Articular cartilage Mankin's score has significant difference in two groups (P<0.01). In groups C and D, at the beginning of the experiment, ED-1 and CGRP in two groups are gradually rised, and reached the peak until the two weeks (group D is lower than group C), then gradually decline, at eight weeks ED-1 in groups C and D are 60.2 and 32.7, respectively. There are significant difference between two groups (P=0.006), at eight weeks CGRP in groups C and D are 6.5 and 3.2, respectively. There are significant difference between two groups (P=0.003). These results showed that Yunnan baiyao with intra-articular injection has a stronger effect for bone and joint.5. Intra-articular injection of Yunnan Baiyao has obvious therapeutic effects and advantages, which may result from the inhibition of CGRP expression and decrease in the number of ED-1 positive inflammatory cells.
