Aims: To investigate the relationship between the incidence of post stroke depression and the location,course as well as type of stroke,and to analysis the effect of cognitive behavior therapy(CBT)on emotion and cognitive of patients with post stroke depression(PSD)by case-control study.Methods: A total of 291 cases of stroke patients completed the general information survey.64 patients with PSD were selected according to the study criteria,and then they were randomly divided into study group(31 cases)and control group(33 cases).The study group received sertraline combined with cognitive behavioral therapy for 9 times totally:cognitive behavioral therapy were taken two times a week for the first four weeks,and then for every two weeks,while the control group only treated with sertraline.Both of the two groups were assessed with DAS scale and HAMD(17 items)scale before and after the treatment.Statistical processing of the results were completed using SPSS17.0 software,with the standard that P < 0.05 for the difference has statistical significance.(?=0.05)Results: 1.The incidence of PSD in our study is about 41.58%,among these PSD patients,about 17.9% with mild depression,22.34% with moderate depression,and 1.37% with severe depression.2.There is no significant difference in the incidence of PSD among different types of stroke and parts of strike patients(P>0.05).3.The incidence of PSD in recovery phase(2-6 months)is higher than that in acute phase(24h to 1 months)(P =0.021 X2=5.337).There was no statistical difference between other stages(P>0.05).4.The HAMD score of the study group and the control group before and after treatment were significantly different(P<0.01).After treatment,the HAMD total score of the study group and the control group are statistically significant(P<0.05).5.92.19% of the DAS total score of the patients with PSD was more than 130 scores.After 6 weeks of treatment,the total score(F=0.53,P=0.04)and factor D1 vulnerability(F=0.54,P=0.04),factor D6 dependent(F=0.49,P=0.03)between the two groups have statistical differences.Within group comparision,the total score(t=2.04,P=0.00),factor D1 vulnerability(t=2.04,P=0.00),factor D6 dependency(t=2.04,P=0.00)of the study group have statistically significant differences before and after treatment.There is no significant difference in the total score of the control group before and after treatment(P > 0.05).Conclusions: 1.The incidence of PSD is high,mainly mild to moderate PSD.2.The course of stroke has an effect on the incidence of PSD,and the recovery period is more likely to suffer from PSD.3.The type of stroke has no significant effects on the incidence of PSD.Also,the location of stroke has no significant effect on the occurrence of PSD.4.Most PSD patients had dysfunctional Attitudests(normal DAS score is less than 130).Drugs and drugs combined with cognitive behavior therapy can improve the symptoms of depression of PSD.The combined use of drug and CBT can improve the emotional symptoms and cognitive function of patients with PSD more effectively,and comprehensive treatment measures should be taken early.
