With the development of social, economic level, the quality of our residents' life has also been improved. At the same time, risk factors such as people's lifestyle and environmental lead to more and more chronic diseases every year. Chronic disease brings individuals and society a heavy economic burden, and also makes the quality of chronic patient's life low. Prevention, combining prevention with treating is a fundamental method to curb epidemic of chronic diseases. In the course of chronic disease prevention individual patient's literacy plays an extremely important role. Therefore, the research on studying the prevention and treatment of chronic diseases from the perspective of health literacy has profound practical significance.The study constructs the connotation of health literacy in patients with chronic diseases: informative health literacy, convictive health literacy, functional health literacy, and behavioral health literacy. Construct evaluation index system of health literacy in patients with chronic diseases through Delphi expert consultation and use fuzzy comprehensive evaluation method to obtain the corresponding index weights. Design questionnaire according to the constructed index system, and investigate Nanjing chronic patients. Construct the structural equation model chronically ill health literacy, and use AMOS software to analyze the relationship between the four connotation of health literacy in patients with chronic. Analyse the impact of these population sociology discrete variables on health literacy in patients with chronic by single factor analysis of variance. Constructing evaluation Index System of health literacy in patients with chronic, and assessing the ability of chronic disease prevention can play an important role in improving the ability of patients with chronic.
