| Objective:On the basis of domestic and foreign health literacy and medical compliance related research,construction of health literacy and health compliance assessment scale.Using survey questionnaire and data collection to study the influence of health literacy on medical compliance and the influencing factors of health literacy and medical compliance in patients with chronic diseases.According to the data analysis results,combined with the existing research results,to explore the intervention measures to improve the health literacy of chronic disease patients.Methods:Two rounds of Delphi were used to construct the scale of health literacy and medical compliance.Using cluster random sampling method,sending out questionnaires to patients with chronic diseases in three cities in Jiangsu province,to verify the reliability and validity of the two assessment of health literacy and medical compliance scale.The data from the survey were recorded by Epdata3.01 software,and the software of SPSS 19.0 was used to analyze the statistical data.Cronbach's alpha coefficient was used to analyze the internal reliability of the scale,and the factor analysis was used to test the validity of the evaluation index system.Through a small range of exploratory investigation,evaluation and development of the evaluation indicators and the availability of possible data sources and data acquisition program.Sample survey by multi stage stratified sampling,taking Jiangsu Province as an example to research in three areas of South of Jiangsu,central Jiangsu and north of Jiangsu as the primary sampling unit.Considering the local policy,economic level,geographical distribution and other factors of the sample,respectively,in Nanjing City,Taizhou City and Xuzhou City,and then in each sample City equal size sample extraction.The survey was 18 years old and above in the urban and rural resident population of chronic diseases,the number of people surveyed was 680.Using the frequency,standard deviation,rate and other statistics to describe the basic situation of the study object;using structural equation model analysis of the correlation between health literacy and medical compliance of patients with chronic diseases;demographic characteristics,using ?2 test and logistic regression analysis of health literacy and health related index influence factors.Results:(1)Health literacy and Medical Compliance Scale Construction:chronic disease health literacy scale(4 dimensions and 41 items)were constructed by two rounds of Delphi and medical adherence scale(4 dimensions and 33 items).Reliability test using Cronbach's alpha coefficient,and Cronbach's coefficient of health literacy scale and each dimension were 0.965.0.947,0.861,0.946,0.938,Cronbach's coefficient of Medical Compliance Scale and each dimension were 0.975,0.960,0.940,0.968,0.947.Using principal component factor analysis to test the validity.Health literacy scale of KMO was 0.952,X1,X2,X3,X4 total of 4 factors respectively from the knowledge,beliefs,function and behavior four aspects to explain the meaning of health literacy,and explained 69.892%of the variance of variation.Medical Compliance Scale KMO value is 0.950,the contentation of medical compliance interpretation 77.167%extract four common factors Y1,Y2,Y3,Y4 respectively from the self monitoring of compliance,compliance,compliance and compliance behavior change.(2)Health literacy and medical compliance of the results of correlation analysis:structural equation model was tested by fitting,and know-ledge,beliefs,function,behavior of health literacy on medical compliance of the correlation coefficients were 0.19,0.23,0.17,0.44.(3)AThe medical compliance of univariate chi ?2 test results show that the area,age,average family income,education level,occupation,health care,health status,duration of disease and the traffic situation of 9 factors was significant.Logistic regression analysis showed that age,area,traffic conditions are factors influencing the medical compliance.nalysis results of influence factors of health literacy and medical compliance:univariate chi ?2 test showed that there was statistical significance in age,average family income,education level,occupation,health care,health,transportation 8 factors;logistic regression analysis showed that region,age,education,health and traffic conditions the P values were less than 0.05.At the same time,the influence factors of single factor and multiple factors were analyzed respectively,and the results showed that the influencing factors of knowledge of health literacy and health literacy of beliefs are area,age,average family income,education level and the traffic situation;influencing factors of functional health literacy for the area,culture degree and traffic behavior;health literacy factors included age,family population,cultural degree health and traffic conditions.Conclusion:(1)The health literacy and medical compliance scale of patients with chronic diseases have good reliability and validity,which can be used to evaluate the health literacy and medical compliance of patients with chronic diseases;(2)The correlation between health literacy and medical compliance,and the time of health literacy;(3)Region,age and traffic condition are the influencing factors of medical compliance.Area,age,education level,occupation,health status,traffic situation is the influence factors of health literacy of chronic disease patients.At the same time,the dimensions of health literacy also have different influencing factors. |
