Objectives To evaluate the prevalence,associated risk factors and impact on general and disease-specific quality of life (QOL) of Urinary incontinence(UI) in patients with chronic obstructivepulmonary disease(COPD).Methods A cross-secontional study was conducted among 315 COPD patients in respiratory medicine ward in three class 1, Grade 3 general hospitalsand two Rank Two, Grade Ageneral hospitals in Jinan and one class 1, Grade 3 general hospital in Dongying of Shandong province from August to December 2015. We used CCI to evaluate the disease comorbidity in patients. We used international incontinence advisory committee on urinary incontinence questionnaire summary table (ICI-Q-SF) to evaluatethe urinary incontinence symptoms. We used SF-36 scale and CCQ scale to estimate the general and disease-specific quality of life of the COPD patients. Data analysis was conducted with SPSS 19.0.Results1. The prevalence of urinary incontinence in COPD Patients, female COPD patients and male COPD patients was 34.6%,50.0% and 27.4%, respectively. Urge urinary incontinence, stress urinary incontinence and mixed urinary incontinence was accounted for 27.5%,39.4% and 33.0%, respectively. Mild, moderate and severe urinary incontinence was accounted for45.9%,38.5% and 15.6%, respectively.2. Multivariate analysis showed that gender, duration of COPD and average daily water consumption were significantly associated with UI.3. The mean of CCQ subscale, PCS subscale and MCS subscale were 3.36±1.03,32.68±8.89 and 41.99±10.85.The general QOL score of patients with UI was significantly lower than patients without UI.4. After adjusting for covariates including age, gender, COPD- related variables and comorbidities, patients with UUI and MUI had lower PCS scores than patients without UI, and patients with UUI and SUI had lower MCS scores than patients without UI. The severity of UI was significantly associated with general QOL and CCQ scores.Conclusions1. The prevalence of UI in COPD patients is higher than that of previous studies conducted in general population in China, and the prevalence in female is higher than male. So the doctors and nurses should pay attention to UI in COPD patients in order to prevence and managements early.2. Gender, duration of COPD, average daily water consumption are independent risk factors for UI in COPD patients. So, for those who are female, longer COPD duration and drinking too much daily water, we need take more attention to them.3. UI negatively affect the QOL of COPD patients, and the effect of different types of UI seems to differ. The severer symptom of UI was associated with worse QOL. It revealed that we should take intervention to those patients to improve their quality of life.
