Background This study reports the prevalence and associated risk factors of diabetes mellitus and impaired glucose regulation in the first large survey conducted among the rural population of Gansu province in Gansu province.Method A multistage cluster random sampling strategy was used to select study participants from the rural locations. The participants were selected from 14 cities in Gansu province who were required lived in local area 5 years and above aged 20-74 years old. All participants need to sign a inform consent. Information collection was using standard questionnaire, with followed by a physical examination and blood biochemical examination. If the investigators without diabetes history, they need undergone standard OGTT test. According to WHO 1999 diabetes diagnostic criteria for diagnosis. Using SPSS 19.0 package for data statistic analysis.Result A total of 14480 individuals(men 6417, women 8063) completed for the study, giving a respond rate 89.4%.The prevalence of diabetes mellitus was 9.5%(10.8% for man and 8.5% for women);age-standardized prevalence was 7.9%.The prevalence of impaired glucose regulation was 16.3%(16.6% for man and 16.1% for women),age-standardized prevalence was15.5%.The prevalence of diabetes mellitus and impaired glucose regulation were different in different age and race groups(P<0.05). The awareness,treatment, control and treatment control rates of diabetes mellitus were low. Finally, The results of logistic regression analysis reveals that age, sex, diabetes history, smoking, overweight, economic development, hypertension, dyslipidaemia were the risk factors of diabetes mellitus; age, sex, smoking, low education level, overweight, hypertension were the risk factor of impaired glucose regulation.Conclusion The prevalence of diabetes mellitus in rural Gansu province in northwest China was higher than expected but lower than national level. The prevalence of diabetes mellitus and impaired glucose regulation increased significantly with age. The prevalence of diabetes and impaired glucose regulation were higher in men than that of in women. The awareness of diabetes in rural areas of Gansu province was weak, diabetes health education is urgently needed. Furthermore, mellitus and impaired glucose regulation in rural settings were largely untreated, appropriate actions need to be taken to provide access to early treatment in order to reduce associated complications.
