Objective: To study the expression rate of CD4+CD25+ Foxp3+Treg and the levels of Caspase-1, IL-1 and IL-10 in serum in type 2 diabetes mellitus complicated with coronary heart disease(CHD); To confirm the role of CD4+CD25+Foxp3+Treg and related cytokines in type 2 diabetes mellitus complicated with coronary heart disease(CHD) and provide evidence for the molecular immune mechanism of the disease.Methods:To detect CD4+CD25+Foxp3+Treg percentage in 20 cases of type 2 diabetic patients with coronary heart disease(CHD), 18 cases of simple type 2 diabetes group and 15 cases of healthy blood by the flow cytometry; To detect caspase-1, IL-1 and IL-10 expression level in serum of the 45 cases of type 2 diabetic patients with coronary heart disease, 40 cases of patients with simple type 2 diabetes mellitus and 36 cases of normal human serum by ELISA; To detect the level of Glu by hexokinase method; To detect the level of Glu by hexokinase; To detect the level of TC by enzyme method; HDL-C by direct method; To detect the level of LDL-C by selective inhibition method.Results: The number of CD4+CD25+Foxp3+Treg in patients with type 2 diabetes mellitus patients with coronary artery disease and simple type 2 diabetes mellitus patients were significantly lower than the control group(P 0.05); the rate of CD4+CD25+Foxp3+ Treg in type 2 diabetic patients with coronary heart disease group is slightly lower than that of simple type 2 diabetes group(P>0.05); The levels of Caspase-1, IL-1?, TC and LDL-c in type 2 diabetes mellitus with coronary heart disease group are higher than that of simple type 2 diabetes mellitus group(P 0.05); The levels of IL-10 and HDL-c in type 2 diabetes mellitus with coronary heart disease group lower than the simple type 2 diabetes group and the control group(P 0.05); The levels of Caspase-1, IL-1?, GLU, TC and LDL-c in simple type 2 diabetes group are higher than that of the control group(P 0.05),but the levels of IL-10 and HDL-c are lower than the control group(P 0.05); the expression rate of CD4+ CD25+Foxp3+Treg in type 2 diabetic patients with coronary heart disease group and simple type 2 diabetes group were positively correlated with the level of IL-10(P 0.05), but were negatively correlated with Caspase-1 and IL-1?(P 0.05).Conclusion: The level of Caspase-1 and IL-1 in CHD group is higher than simple T2 DM group and control group, indicating that T2DM+CHD patients and simple T2 DM patients have more severe inflammatory reaction. The expression rate of CD4+CD25+Foxp3+Treg in type 2 diabetes mellitus patients with coronary heart disease group and simple type 2 diabetes group decreased, and it was negatively correlated with the level of Caspase-1, IL-1, suggesting that the occurrence of the disease associated with immune disorders. The expression rate of CD4+CD25+Foxp3+Treg and the level of IL-10 expression in Type 2 diabetes mellitus complicated with coronary heart disease group and simple type 2 diabetes mellitus group patients were significantly lower than that of the control group, showed that CD4+CD25+Foxp3+Treg decreased and the number of secreting IL-10 reduced, the negative immune regulation function reduced, resulting in a large number of inflammatory factors released and inflammatory injury This shows that immune dysfunction may be the key factor of T2 DM complicated with coronary heart disease.
