Objective Perfect the parathyroid scoring index in the Thyroid operation, verify relationship of parathyroid index and postoperative parathyroid function further.Evaluate the function of the parathyroid gland in Thyroid surgery by this system.Methods Perfect the parathyroid index in suit and the parathyroid transplantation index.In the same surgical team, Papillary Thyroid Carcinoma confirmed by pathology in 59 cases between March 2015 and February 2016 were retrospectively analyzed.There are 17 male patients(28.8%),and 42 female patients(71.2%).the age of patients is between 18 ~ 67, the average age is(43.25 ± 11.33). According to the wide of the Level VI Neck Dissection in surgery we Divided them into two groups.Group A take the unilateral lobectomy and the tumor side Level VI Neck Dissection,and group B Take the unilateral lobectomy and the double side Level VI Neck Dissection. Monitoring the preoperative serum parathyroid hormone,calcium and postoperative 12-24 hours of parathyroid hormone,calcium, serum parathyroid hormone and calcium after two weeks. According to the 2 weeks after operation, serum parathyroid hormone(PTH2) in the preoperative serum parathyroid hormone(PTH0) ratio,we assess the damage of parathyroid function. According to the parathyroid index, we obtain index difference after operation treatment finally.Using SPSS 19.0 statistical software for data analysis, count data use ? 2 test, measurement data use t test, P < 0.05 for the standard with statistical difference. The ratio of postoperative serum parathyroid hormone and serum parathyroid hormone after two weeks(PTH2/PTH0) is for the Y axis, and the difference of parathyroid index is for X axis.Describe a scatter diagram.Both of them are analyzed by the SPSS19.0 statistical software, results suggest that relevance, which P < 0.05. Use receiver operator characteristic curve(ROC) process. The difference of parathyroid index is for the test variables,The result of hypoparathyroidism which is defined by serum parathyroid hormone after two weeks is state variables for the analysis of ROC curve, P < 0.05 for the standard with statistical difference.Results 1.After two weeks the serum PTH, serum calcium and preoperative serum PTH, serum calcium is analyzed by paired t test, there is statistical significance(P < 0.05).2. The ratio of postoperative serum parathyroid hormone in two weeks and the serum parathyroid hormone before operation(PTH2/PTH0) and parathyroid index difference of the linear regression analysis, results suggest that relevance,which R=0.460, P<0.05.3.Use the same method we can count R= 0.522,p<0.05 in group A;and R=0.358,P<0.05 in group B.The area under the ROC curve was 0.746. P=0.009<0.05.which means that evaluate the function of the parathyroid gland in thyroid surgery by this system accord with practice.Conclusion 1. The operation of is bound to have an impact on parathyroid function.2. The intraoperative parathyroid index has medium diagnostic value of predicting the parathyroid function after Papillary Thyroid Carcinoma surgery.And it`s benefit for clinical doctors to make the preoperative assessment of the second time surgery patient in the future.3.The intraoperative parathyroid index of the inferior parathyroid gland need to be improved with further clinical research.And we should broadened indication range of the Parathyroid transplantation.
