| Objective:Hypoparathyroidism is a serious complication of thyroidectomy.In this study,we observed the levels and changes of parathyroid hormone in serum and drainage fluid after total thyroidectomy.And then we compared the predictive effect of different methods on hypoparathyroidism after total parathyroidectomy..Methods:130 patients of total thyroidectomy were selected.The basic conditions of the patients were recorded,and the serum levels of PTH and calcium were measured at first day,third day and 6 months after the operation.The concentration of PTH in the drainage fluid was measured at first days and third days after operation.We compared the predict of prognosis of postoperative hypoparathyroidism between the serum level of PTH on the first day and third day after operation and the change of PTH in drainage fluid.Results:On the 1 day after operation,serum PTH decreased significantly compared with before operation and the difference was statistically significant(10.36 vs 39.91,P <0.001).Blood calcium level decreased significantly on the 1 day after operation,and the difference was statistically significant(2.07 vs 2.40,P < 0.001).Change of drainage fluid PTH(986.96 + 232.264 vs653.54 + 128.700,P<0.001),first day after operation(18.34 +7.795 vs 13.27 + 7.168,P<0.05),third day after operation(30.94 vs 14.9,P<0.001)of serum PTH level were statistically different.On the first day after operation,the area under curve(AUC)was 0.695,and the third day after operation was 0.648.The AUC ofchange of PTH in drainage fluid was 0.889.The prediction of the drainage fluid PTH was better,and the difference was statistically significant.Conclusion:After the total thyroidectomy,the level of PTH in the drainage fluid has a stronger predictive value for hypoparathyroidism than PTH on the first and third day after operation.It can help us to better evaluate the prognosis of patients. |
