The Medical Service Ability Evaluation And Influence Factors Analysis On County General Hospital In Fujian

Objective: Through literature review and study relevant policy documents,define the function orientation of county-level hospitals;and to evaluate the Fujian province county hospital medical service ability;and to analyze the related factors influencing the service ability,for the administrative department to make the decision that promotion the service ability more scientific and reasonable.Methods:According to the questionnaire survey data, using descriptive statistics to analyze the composition of Fujian province county level hospital human resources and medical service ability,using Lorenz curve and Gini coefficient to analyze the fairness of health resource allocation situation;Using Analytic Hierarchy Process(ahp) to comprehensive evaluation the medical service ability,at the same time,using multiple linear regression method to analyze the factors that affect hospital medical service ability.Result:1.In terms of the sickbed,the 62 county hospital have 25058 sickbed at the total,each hospital have 404 sickbeds on average;the actual open sickbeds 27809,each hospital have 449 sickbeds on average.23(37.1%) hospital have more than 500 sickbeds,yet reached 500 sickbeds have 39(62.9%).2.In terms of the quantity,the outpatient and emergency of the 62 hospitals is 20.11million;the discharge is 1.1656 million;and the in hospital surgery is 228400.Each hospital year discharges have an average of 18800 people,3684 in the hospital surgery;the average operation and operation discharges percentage is 19.60%;the kinds of common disease that should be carried out is 233,actually 163(69.18%);the kinds of the key diagnosis and treatment technology or surgery is 206,the actual is 61(28.33%).3.In terms of the efficiency and quality,the 62 hospital beds in working days is 62,the average sickbed utilization rate is 98.34%,and the sickbed turns is 43.38 times,the dischargee stay in the hospital average in 7.2 days.the Outpatient and admitted to hospital diagnosis coincidence rate is 96.53%,admitted to hospital and discharge diagnosis coincidence rate is 97.43%,the diagnostic before and after the operation coincidence rate is 97.79%,the clinical and pathological diagnosis coincidence rate is 92.18%,the critically patients rescue success rate is 91.23%,the inpatient mortality is 0.27%,and surgery in hospital mortality is 0.01%,the emergency mortality rate is 0.18%,sterile surgical infection rate was 0.26%,the incidence of nosocomial infection is 0.81%.4.About the information system,all 62 hospital have HIS system,54 have LIS system(87.10%),51 have PACS system(82.26%),40 have EMR system(64.51%),8 have Remote medical system(12.90%).5.According to the evaluation results,Fuzhou,Quanzhou county hospital medical service ability evaluation result is good;Longyan,Ningde,Putian and Nanping county hospital medical service ability evaluation result is middle;the Zhangzhou,Sanming county hospital medical service ability evaluation is poor.Conclusion:The survey results show that the Fujian Province county hospital human resources exist some deficiencies,the excellent professional and technical personnel is relatively lack,the education and professional title irregularity.Using the lorenz curve and gini coefficient to analysis the health technicians and beds according to the population distribution,and the results show that the allocation is relatively reasonable,basic in the average state.According to the comprehensive evaluation,Fuzhou,Quanzhou county hospital medical service ability evaluation result is good;Longyan,Ningde,Putian,Nanping county hospital medical service ability evaluation result is middle;Zhangzhou,Sanming,county hospital medical service ability evaluation is poor.Through multiple linear regression analysis shows that the ten thousand yuan of above equipment,bed has an impact on the county level comprehensive hospital medical services.
