Study Of Evaluation Index System On The Management Of Disinfection And Sterilization Of Medical Devices In Oral Clinic

Objective: To protect the safety of patients and doctors, and reduce the risk of disease transmission during oral treatment, we explored the evaluation index on management situation of disinfection and sterilization of oral medical devices through the Delphi method and determined the index weight on the basis of investigation on current infection management situation of Zunyi stomatological hospital, used Weighted Rank Sum Radio( WRSR) method to evaluate and classify current management situation of disinfection and sterilization of medical devices in Zunyi oral medical and health institutions comprehensively, to explore the methods to improve the management level of disinfection and sterilization of medical devices in the oral clinic. Methods: From October to December 2014, 127 medical institutions which carried out oral treatment at different levels in 14 districts(county, city) of Zunyi as the object of investigation, we use self-made questionnaire to carry out the census on current infection management situation of oral medical and health institutions.Oral medical quality control center of Guizhou Province organized personal training, and the survey about management of disinfection and sterilization of medical devices are the general situation of oral medical and health institutions and current management situation of disinfection and sterilization of medical devices.According to expert consultation, using analysis hierarchy process to calculate the weight coefficient of each evaluation index, using WRSR to evaluate and classify the current management situation of disinfection and sterilization of medical devices in Zunyi 127 oral medical and health institutions comprehensively. Results:? Distribution of Zunyi oral medical and health institutions: to the end of the investigation, there are 127 oral medical and health institutions, including 45 departments of Stomatology, accounting for 35.43%, and 76 dental clinics and outpatient departments, accounting for 59.84%.?This study establish the evaluation system on management situation of disinfection and sterilization of oral medical devices through literature research, related rules and regulations analysis and panel discussion. The evaluation system includes 4 aspects and 29 indicators, respectively is institution management, quarantine measures, management of disinfection and sterilization, and disinfection and disposable supplies management.?The management situation of disinfection and sterilization of oral medical devices is not high in Zunyi: total scores(0.36 + 0.23), 9 institutions scores less than 0.10, accounting for 7.09%, 3 among them scores 0; 19 institutions scores more than 0.60, accounting for 14.96%.?According to WRSR, the management situation of disinfection and sterilization of oral medical devices in Zunyi is divided into high, medium and low grades, the low grades included 11 institutions and all of them are oral clinics, all “scaling shops” are low-grade; the high-grade includs 23 institutions, and the rest is the medium grades, includs 93 institutions.?In the aspect of infection management in oral hospital, the rules and regulations should be formulated in line with the development of all levels of the institutions under the hierarchical management philosophy; and equipped with full time or part time personnel specialize in disinfection and sterilization, the equipment which operated in oral cavity of the patient should be "a man with a sanitized or sterilized", and used the timely cleaning, appropriate disinfection or sterilization of the used equipment as mandatory access standard for all levels of the institutions. Conclusion: The difference of the management situation of disinfection and sterilization of oral medical devices is obvious, but overall is not high; Expert consultation method and analytic hierarchy process could applied for the construction of the evaluation index on management situation of disinfection and sterilization of oral medical devices; the classification on the management situation of disinfection and sterilization of oral medical devices in Zunyi through WRSR conforms to the reality, health administrative departments should ban the low-grade oral clinics, intensify the supervision of medical institutions in the mid-grade.
