Objective : By exploring initially the location and properties distribution of melancholia as well as syndrome differentiation principles, to provide some differentiation basis for clinical diagnosis and treatment of melancholia.Methods:Referred to study literature of melancholia, we sorted out, analyzed and reviewed the latest research progress of causes and mechanism of melancholia by modern medicine. At the same time, melancholia cases in literature were gathered and the general data and performances of 208 cases with complete information were recorded. And we input all information, by two persons, into Excel and analyzed that with Frequencies and Clustering Algorithm,and explored distribution features and syndrome differentiation principles of melancholia.Results: The location distribution of melancholia was listed as: liver(69.7%), heart(46.2%), spleen(30.8%), kidney(13.0%), gallbladder(7.2%), stomach(6.3%), lung(1.4%); while the properties distribution of melancholia were listed as: Qi stagnation(56.3%), Qi deficiency(34.6%), phlegm(20.7%), Yin deficiency(13.5%), endogenous fire(11.5%), blood deficiency(6.3%), blood stasis(5.8%), dampness(4.8%), Yang deficiency(3.4%), Qi reversion(2.4%), essence deficiency(1.9%). As a result, syndromes of melancholia were listed as Liver-qi stagnation(22.6%), Heart-yin deficiency(19.2%), Qi stagnation and phlegm retention(15.9%), Qi depression transforming into fire(15.9%), both deficiency of heart and spleen(14.4%), Liver-qi depression and spleen deficiency(12.0%).Conclusion: Melancholia is generated from depression, angry, excessive thoughts, pessimism and worry that cause malfunction of heart, liver, spleen as well as disorder of zang-fu, qi-blood and yin-yang. During the initial stage, excess syndrome can be seen for qi-stagnation can cause phlegm, food and heat retention. As the diseases prolong, the excess can transform into deficiency syndromes with depression hurting the heart, deficiency of qi and blood of heart and spleen as well as yin-deficiency with exogenous fire. Intermingled excess and deficiency syndrome is also common to see. Common syndromes are listed as liver-qi depression, heart-yin deficiency, qi stagnation and phlegm retention, qi depression transforming into exogenous fire, both deficiency of heart and spleen as well as liver depression and spleen deficiency.
