Purpose: To evaluate the efficacy and safety of percutaneous endoscopic lumbar disectomy(PELD) for the treatment of symptomatic lumbar disc herniation.Method: This prospective interventional case series recruited patients with SLDH who were underwent PELD from Chenzhou NO.1 People's Hospital and Qixing Hospital in Chenzhou. Transforaminal surgical approach in 19 patients(group A), and translaminar approach in 13 patients(Group B).All patients were followed up regularly evaluate of clinical efficacy at least 6 months, 1 week, 1-month, 3-month, 6-month respectively, and pain at pre-operation and post-operation was assessed using a visual analog pain scale(VAS). Other postoperative outcomes included Macnab's criteria, Oswestry disability index(ODI) score, changes of MRI parameters and complications.Results: Thirty-two patients were included in this study, including 14 male(43.8%) and 18 female(56.2%), with mean age 45.9±11.9 years. The mean weight was 65.75±10.83 kilogram(ranged from 54 to 85 kilogram). The mean duration of SLDH ranged 1 to 12 years(mean, 5.42±1.57 years). After PELD, the back pains were relieved or disappeared rapidly. The mean preoperative VAS was 7.8, and decreased to 2.5 post-operation. The mean VAS at 1 month, 3 months, and 6 months after operation were significantly lower than that at pre-operation. Similarly, ODI scores improved at 1 month after operation, and the ODI scores increased to levels at healthy population at 6 months after operation, compared with those of VAS and ODI, and there were no significantly difference between two kinds of surgical treatment for lumbar disc herniation(P>0.05).Conclusions: PELD is a safe and effective method for the treatment of lumbar disc herniation, and the clinical efficacy in two groups of surgical treatment is similar. It has advantages of fewer traumas, quick recovery, and so on. It is suggested that PELD treatment of lumbar disc herniation is worth promoting.
