| Aim: To investigate the Serum uric acid and its association with acute cerebral infarction and carotid atherosclerosis in hypertensive patientsSubjects and methods: A total of 299 hypertensive patients who from the First Affiliated Hospital of University of South China from January 2014 to January 2015 was selected by randomly receiving the surveys.According to the occurrence of acute cerebral infarction(ACI),we divided the 299 patients into two groups:hypertension with ACI(199 cases) and hypertension without ACI(100 cases). Age,gender,body mass index(BMI),smoking,diabetes,coronary heart disease,Hb A1 c,total cholesterol(TC),triglyceride(TG), high density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol( LDL-C), serum uric acid(SUA), carotid artery- media thickness(CIMT), The detection rate of carotid artery plaque were compared. The factor which was considered statistically significant would be analysied by logistic regression technique. The 199 cases of ACI were divided into study group,which is carotid atherosclerosis(CA,171 cases) and normal carotid group(28 cases). Age, gender, BMI, smoking, diabetes, coronary heart disease, Hb A1 c, TC, TG, HDL-C, LDL-C,SUA were compared.The factor which was considered statistically significant would be analysied by logistic regression technique.Then,199 cases of Hypertension with ACI were categorized into quartile based on the SUA level. Age, gender, BMI, smoking, diabetes, coronary heart disease,Hb A1 c, TC, TG, HDL-C, LDL-C, CIMT, The detection rate of carotid artery plaque were compared。Result: 1.The proportion of the male who complicated with Hypertension and ACI is 67.3%, BMI(24.49 + 3.19), proportion of diabetes(26.1%), the proportion of smoking(60.8%), Hb A1c(6.73 + 1.81), SUA(327.52 + 101.27), CIMT(1.07 + 0.21).All above were obviously higher than that of the control group and had statistical difference(P < 0.05); 2. In Hypertensive patients, BMI(β= 0.798, OR=2.222), Hb A1c(β=1.651 OR=5.211), CIMT(β=1.269, OR=3.556), plaque rate(β=0.616, OR=1.852)is positively correlated with ACI. Female(β=-0.741, OR=0.477) is negatively correlated with ACI. 3.Comparison of carotid atherosclerosis and normal carotid artery in hypertension complicated with ACI. The smoking and SUA were statistically significant difference(P < 0.05).smoking(β=0.801, OR=2.228), SUA(β=0.011, OR=1.011) is positively correlated with CA 4. In Hypertensive patients with acute cerebral infarction, there is no significant difference(P > 0.05) between 1, 2, 3 quartile groups in the detection rate of plaque, The detection rate of plaque had significant difference between highest quartile group and other three quartile group(P < 0.05)。Conclusion:1.There is no clearly correlation between SUA and ACI in hypertensive patients; 2.SUA is positively correlated with CA and there is two ladder relationship between SUA and carotid plaque in hypertensive patients with ACI. |
