In recent years the incidence of lobar pneumonia in children increased year by year.Lesions in a wide range,the disease often involves a lobectomy or more lung,clinical symptoms,etiology is more complex,western medicine is given priority to with the original microbial resistance and suit the support therapy.Traditional Chinese medicine in improving children's lobar pneumonia clinical signs and symptoms,shorten the course of the disease characteristics and advantages,has a good application prospect and is worth further study.Phlegm heat indicates pulmonary certificate,toxic heat indicates pulmonary certificate for the disease is often in the acute phase of witness.Qingfei jiedu decoction is our pediatric ward two long-term clinical effective prescription in the treatment of lobar pneumonia in children,but it is the lack of a system,standardization of clinical research.Objective: Observation of qingfei jiedu decoction combined western medicine therapy lobar pneumonia in children with placebo combined western medicine therapy on body temperature,cough,sputum,chest CT,and the change of the respect such as inflammatory indexes and evaluate the clinical curative effect of two different treatments.To observe the blood routine,urine routine,stool routine,liver,kidney and possible adverse events,the safety evaluation of two different treatments.Observe the change of platelet count and coagulation index,confirmed that the children of lobar pneumonia high blood coagulation state,to explore the traditional Chinese medicine syndrome differentiation treatment to add the necessity of using promoting blood circulation to remove blood stasis drugs.Methods:Using the random number table method randomized,control group30 cases,treatment group 60 cases.Control group with placebo with conventional western medicine therapy,treatment group with clear lung jiedu decoction with conventional western medicine therapy.Compare two groups before and after treatment in children with body temperature returned to normal time,cough,compare two groups of symptoms and signs index,infection and lung CT to improve the situation,the comprehensive curative effect evaluation of two groups.Results: 90 cases,this study case fall off 0 cases,loss rate is 0.00%,excluding 0 cases,eliminating rate was 0.00%.Conform to the solution set of 90 cases,90 cases of safety set.Research before the demographic characteristics,clinical characteristics,distribution and the duration of symptoms integral comparison,there was no significant difference between the two groups,P > 0.05,shows that two groups of comparable.After the research,cured,significant effectivi,improved and ineffective cases of treatment group were 25(41.7%),33(55%),2(3.3%),and 0(0.0%),control group of 4(13.3%),17(56.7%),9(30.0%),and 0(0.00%),after statistics analysis,P <0.05,the treatment group is better than the control group.Treatment group recovery efficiency is 96.7%,control group recovery efficiency is 69%,compared two groups was statistically significant,p < 0.05.Individual clinical data results shown: body temperature,cough time returns to normal,the treatment group is better than that of control group,p < 0.05,the difference was statistically significant.Cough,sputum,improvements in symptoms integral,the treatment group is better than that of control group,p < 0.05,the difference was statistically significant.Lung imaging changes comparing the two groups,P < 0.05,there is significant difference.In the WBC,N,CRP changes,the control group,treatment group,two groups before and after treatment level have significant difference,P < 0.05).The difference between the two groups,P > 0.05,no significant difference.PLT increased cases of treatment group and27 cases,accounting for 45%,the control group(19 cases),accounting for 63.3%.D-dimer rise in cases,31 cases in treatment group,51.7%,the control group,15 cases(50%).Cases and blood stasis,29 cases in 90 cases,accounting for 32.2%.PLT before and after treatment in the two groups,the treatment group after statistics processing,P< 0.05,there is significant difference.After statistics processing,the control group(P >0.05),and there was no significant difference.Test was not observed in the test drug has abnormal effect on blood system and the liver,kidney and other adverse reactions.Conclusion: Qingfei jiedu decoction group lobar pneumonia of children is effective and safe treatment.
