Objective: By means of clinical research and animal experiment,the study is aimed to observe the serum concentration of thyroglobulin antibodies(TgAb) and thyroperoxidase antibodies(TPOAb) as qingying huatan decoction treats the fire excess from yin deficiency and coagulated phlegm of Hashimoto thyroiditis(HT) so that we can evaluate the effect of qingying huatan decoction and have the first understanding of its mechanism of action.Methods:Clinical research:Collect 40 patients who have the fire excess from yin deficiency and coagulated phlegm of HT,and divide the patients into two groups randomly.Each group has 20 patients.According to thyroid function of the patients of the control group,the patients who have hypothyroidism give Euthyrox,and who have hyperthyroidism give Thiamazole,and who have normal thyroid function give lifestyle intervention.The treatment lasts for 3 months.Based on the treatment of the control group,all patients of the experimental group take qingying huatan decoction.Everyone measures the fasting serum concentration of TgAb, TPOAb, free thyroxine(FT4),free triiodothyronine(FT3) and thyroid stimulating hormone(TSH) at prior treatment and 1,2,3 months after treatment and record variation of the serum concentration of TgAb, TPOAb.Animal experiment:There are 72 SPF female Wistar rats and divide into six groups randomly including the blank group,disease model group,qingying huatan decoction group, zhibo dihuang decoction group, Euthyrox group and Tripterygium glycosides group.Each group has 12 rats.Model makes 6 weeks.The rats of the blank group are given basic feed.Other groups are given high-fat diet and high iodine water.Except the control group, the others also are accepted hypodermic with Freund’s Adjuvant antigen emulsion,clamping tails,gavage with the suspension of Hydrocortisone Acetate tablets,and so on.The treatment by gavage lasts for 4 weeks.Every group are given basic feed.The blank group and disease model group are gavage with water.The qingying huatan decoction group,zhibo dihuang decoction group,Euthyrox group and Tripterygium glycosides group are respective gavage with qingying huatan decoction,zhibo dihuang decoction,the suspension of Euthyrox and the suspension of Tripterygium glycosides.Measure and contrast the rats’ serum concentration of TgAb, TPOAb and cycloxygenase(COX)2 by ELISA.Results:Clinical research:1.For the descent of the patients’ serum concentration of TgAb,there is a difference between the control group and the experimental group(P<0.05).For the descent of the patients’ serum concentration of TPOAb, there is a significant difference between the two groups(P<0.01).Compared with the control group,the descents of the experimental group’s serum concentration of TgAb and TPOAb are more obvious.2.For the descents of the serum concentration of TgAb and TPOAb of the patients who have abnormal thyroid function,there are differences between the control group and the experimental group(P<0.05).The descents of the serum concentration of TgAb and TPOAb of the experimental group’s patients who have abnormal thyroid function are more obvious than the control group’s.3.In the experimental group,the descents ofthe serum concentration of TgAb and TPOAb have no differences between the patients who have abnormal thyroid function and the patients who have normal thyroid function(P>0.05).4.In the control group,the descents of the serum concentration of TgAb and TPOAb have no differences between the patients who have abnormal thyroid function and the patients who have normal thyroid function(P>0.05).5.In the experimental group,the descents of the serum concentration of TgAb and TPOAb have no differences between the patients who have hypothyroidism and the patients who have hyperthyroidism(P>0.05).Animal experiment:1.Randomly choose two groups between the disease model group,qingying huatan decoction group,zhibo dihuang decoction group, Euthyrox group and Tripterygium glycosides group,then compare their serum concentration of TgAb,TPOAb and COX-2 one by one.The rats’ serum concentration of TgAb,TPOAb and COX-2 have differences between the Tripterygium glycosides group and the disease model group(P<0.05).The rats’ serum concentration of TgAb,TPOAb and COX-2 have differences between the Euthyrox group and the zhibo dihuang decoction group(P<0.05).Except the above combinations,the rats’ serum concentration of TgAb,TPOAb and COX-2 have significant differences between any two groups(P<0.01).The rats’ serum concentration of TgAb,TPOAb and COX-2 from low to high are qingying huatan decoction group,Euthyrox group,zhibo dihuang decoction group,Tripterygium glycosides group and the disease model group.2.The rats’ serum concentration of COX-2 is positively associated with the rats’ serum concentration of TgAb and TPOAb(0<r<1,P<0.01).Conclusions:Clinical research:Qingying huatan decoction can obviously reduce the patients’ serum concentration of TgAb and TPOAb who have the fire excess from yin deficiency and coagulated phlegm of HT.At the same time,western medicine has little effect on reducing the patients’ serum concentration of TgAb and TPOAb.Whether hypothyroidism,hyperthyroidism or normal thyroid function,the qingying huatan decoction can reduce the patients’ serum concentration of TgAb and TPOAb.So the qingying huatan decoction plays a role in any situation.Thus it can be seen that the traditional Chinese medicine has advantage in treatment based on syndrome differentiation of HT.Animal experiment:Qingying huatan decoction can nourish yin and clear heat,reduce phlegm and resolve masses.It can reduce the rats’ serum concentration of TgAb,TPOAb and COX-2.At the same time,the study speculates the effect of qingying huatan decoction may be connected with reducing the serum concentration of COX-2 thereby alleviating inflammation. |
