ObjectiveThe purpose of this study was to investigate the present situation of the self-directed-learning and the clinical communication competence and analyze the relationships between them, in order to provide reference for educators and managers to cultivate and improve self-directed learning and clinical communication skills of nursing students. MethodsThe survey was used convenience sampling method, in October to December of 2014, choosing 500 nursing interns who have practiced more than 2 months in two First-class Hospitals at Grade 3 of Henan as research subjects. The survey tools is consisted of three parts, including general information questionnaire, Self-Rating-Scale for Self-Directed Readiness Learning(SRSSDL) and nursing students' clinical communication skills assessment scale.The data base was established by EpiData3.2, and SPSS 18.0 statistical software is used for data analysis. Bivariate is performed by t test and single factor variance, the multivariate was adopted by hierarchical regression analysis, and the correlation analysis. Results1. The situation of nursing interns' self-directed learning and clinical communication skillsNursing interns' self-directed learning and five dimensions mean scores were showed as below: the total score of self-directed learning was(223.82±29.03), the sense of learning(44.87±6.19), learning strategies(44.42±6.65), learning behavior(43.51±6.68), learning evaluation(44.48±6.96), interpersonal skills(46.54±6.82). The distribution of self-directed learning's three levels was as follows: 1 students with poor self-directed learning scores accounted for 0.2%; 205 students with general self-directed learning scores accounted for 44.4%; 256 students with the best self-directed learning scores accounted for 55.4%.The total score of clinical communication skills was(86.28±9.38), the establishment of a harmonious relationship(19.07±2.64) points, keen to listen to(16.66±2.61) points, confirm the problems of patient(15.92±2.68) points, mutual participation(10.67±2.46) points, the effective transfer of information(8.77±1.54) points, verification feeling(15.19±2.55) points. According to the grade scale of clinical communication skills, students were classified four levels and the distribution was listed: 6 students with poor scores accounted for1.3%; 203 students with medium scores accounted for 43.9%; 219 students with good scores accounted for 47.4%; 34 students with excellent scores accounted for 7.4%, nursing students' scoring rate was good level is the most.2. The comparison of nursing interns' self-directed learning on individual characteristicsDifferent gender, family sources, whether only a child, father's education level, mother' education level, father's occupation, mother's occupation were not statistically significant(P>0.05). In addition to the sense of learning, different education degrees and the different relationship with their parents on the rest of the dimensions were statistically different; different personality types of nursing interns in learning strategies, learning behavior, SRSSDL scores were significantly different; whether students are leader in learning awareness, learning behavior, learning evaluation, SRSSDL score was statistically significant; whether they like nursing, curriculum theory scores, results of clinical skills, the relationship with students, the relationship with teachers and clinical education teachers were significantly different(P<0.05) in all dimensions and SRSSDL score.3. The comparison of nursing interns' clinical communication ability on individual characteristicsThe clinical communication ability was statistically significant different within groups with different curriculum theory scores, the results of clinical skills, peer relationships, the relationship with teachers, clinical teachers, the parents(P<0.05).The higher the theoretical and clinical operation skill score, the stronger the clinical communication competence; The clinical communication skills in the relationship with students, teachers, clinical teachers and their parents that better relation group higher than good relation group, the general group; The clinical communication competence in the relationship with teachers that better relation group higher than good relation group.4. The relationship between nursing students' self-directed learning and clinical communication skillsThere are positive correlation between self-directed learning, dimensions(awareness of learning, learning strategy, learning behavior, learning evaluation, interpersonal skills) and clinical communication skills(r=0.350~0.487, P<0.05 or P<0.01); Hierarchical regression analysis showed that personality type, the relationship with teachers, the relationship with clinical teachers, the relationship with the parents, self-directed learning and its all dimensions are the significant factors of clinical communication skills. Among them, self-directed learning were to predict the variation of 25.1%(R2=0.278, F=9.453). Conclusions1. The self-directed learning score of nursing students is in the middle level, still needing to be improved. Nursing educators and administrators can improve interns' self-directed learning according to different individual characteristics.2. Nursing students' clinical communication skills overall level is not high, Interventions aimed to improve its level can be adopted from nursing interns' personality types, the relationship with teachers, clinical teachers and the parents and so on.3. The nursing interns' self-directed-learning can positively predict their clinical communication competence, which points out that cultivating and improving nursing students' self-directed learning can help to improve their clinical communication skills in general.
