Objective:To detect the expression levels of CD25,CD69 and HLA-DR on the CD4~+T lymphocytes surface of peripheral circulation in perioperative period of colorectal cancer(CRC).Analyse these changes and explore the mechanism of immune for significance.Methods:Flow eytometry was used to detect the expression of CD25,CD69 and HLA-DR on the CD4~+T lymphocytes surface in peripheral circulation CD4+ T lymphocytes in 40 cases of CRC(Observation group)during perioperative period including the preoperative and postoperative 5,10 days(POD5,10),while 15 persons undergoing health examination(Control group)were involved as control.These changes and these relationships of their expression with clinieo-pathological features was analyzed.Results:1.In both observation group and control group in perioperative period expressed CD25,CD69 and HLA-DR on cell surface of peripheral circulation CD4~+T lymphocytes.2.The expression level of CD25 in observation group before operation was markedly higher than that of in control group(P<0.05),and gradualy decreased after surgery.However,there had no significant difference between pre-operation and POD 5(P>0.05).The expression level of CD25 on POD10 was significantly lower than that in pre-operation and POD 5(P<0.05),but the expression level on POD10 was markedly higher than that in control group still(P<0.05).The expression level of CD25 in observation group of perioperative period had no ob vious correlation with clinic pathological features(P>0.05).3.The expression level of CD69 in the observation group in pre-operation was markedly lower than that of in the control group(P<0.05),and gradualy increased after surgery.The expression level on POD 5 and POD 10 showed no significant difference(P>0.05),but both those were significantly higher than that of in pre-operation(P<0.05).However,the expression level on CD69 on POD10 was still markedly lower than that of in control group(P<0.05).The expression level of CD69 was negatively correlated with the difference of patients' lymph node metastasis and TNM stage before preoperation(P<0.05),but it showed no obvious correlation with other clinieo-pathological features(P>0.05).4.The expression level of HLA-DR in observation group of pre-operation was markedly lower than that of in control group(P<0.05),and gradualy increased after surgery.The expression level in pre-operation and POD10 showed no significant difference with that of POD5(P>0.05).The expression level of POD10 was significantly higher than that of in the pre-operation(P<0.05),but was still lower than that of in control group(P<0.05).The expression level of HLA-DR was positively correlated with the difference of length to diameter of tumor,lymph node metastasis and TNM stage before preoperation(P<0.05).The expression level of HLA-DR was positively correlated with the difference of lymph node metastasis and TNM stage of colorectal in the postoperation(P<0.05).But it showed no obvious correlation with other clinic pathological features(P>0.05).Conclusion:Activation rate of CD4~+T lymphocytes in perioperative period of CRC was low,and with the function of cellular immunity aslo had declined.Tumor immunosuppression might suggest why there was a high expression of CD25 on the CD4~+T lymphocytes surface of peripheral circulation in perioperative period of CRC.The expression of CD25 decreased after operation,might related to the tumor immunosuppression by removal of the tumor.The expression of CD25 had no correlation with clinic pathological features.The level of the expression of C D69 and HLA-DR of CRC were reduced in the preoperation,then recovered gradually after operation.The evidence proved the patients with a weakened immune system in the preoperation and the immune functional was recovered after tumer remove.The relation of clinic pathological features and CD69 and HLA-DR evaluate the prognosis.CD25,CD69 and HLA-DR on CD4~+T lymphocytes surface of peripheral circulation might be a cellular immunological index which could helped to estimate the state of CRC,and,guide the manage of perioperative period and immune therapy in CRC.
