| Objective: Use Meta-analysis to assess the effectiveness of BuYang HuanWu decoction plus conventional western treatment in treating diabetic peripheral neuropathy,to provide clinical evidence of EBM through screening and analyzing of clinical research literature on VCI.Method: Systematic and comprehensive retrieval CNKI,Wanfang Data,VIP and other related databases,collecting clinical research literatures using Buyang Huanwu decoction combine with conventional western medicine treated diabetic peripheral neuropathy,from 2005 to 2015,lit and screened according to the literature selection criteria.Compared the effectiveness of Buyang Huanwu decoction combine with conventional western medicine with who just used western medicine,and made Meta-analysis.Result:(1)According to the requirement of documentation,searching literatures from related databases by keywords,totally searched out 183 articles,31 articles were repeat,then by reading the headline and abstract to screening document,exclude 24non-compliant document,get 128 compliant document.Then read the full text of 128 documents,exclude 87 non-compliant documen,screened out 41 articles at last.All of them include randomized control trial 3526,in which the experiment group is 1826,and the control group is 1700.(2)All of the 41 treatments used BuYang HuanWu decoction(24treatments used only BuYang Huan Wu decoction,17 treatments used BuYang HuanWu decoction combine with other traditional Chinese drugs).All of the 41 controls used western medicines(14 of them only used Vitamin B,12 of them only used Methylcobalamin,2 of them only used Beraprost Sodium Tables,5 of them used Methylcobalamin and Vitamin B,the remained used other western medicines or combined with other medicines).(3)Calculating the treatment in weekly,all of the included studies have the object treatment,1 of the treatment was 2 weeks,4 were3 weeks,21 were 4 weeks,2 were 6 weeks,8 were 8 weeks,3 were12 weeks,1 was 16 weeks,1 was 12-24 weeks.(4)Comparison between Buyang Huanwu decoction plus western medicine and western medicine in DPN[OR=4.26,95%CI(3.56,5.09),Z=15.93,P<0.00001]?(5)According to the different of Western medicines,classification all the researchers to 5 subgroup,combine the Meta-analysis result of 5 subgroup,the MD[95%CI] were 4.22[3.05,5.84]?3.92[2.89,5.32]?5.55[3.21,9.59]?7.67[2.38,24.65]?3.99[2.73,5.81],it can be concluded that the result of Buyang Huanwu Decoction combined with Western medicine treatment of diabetic peripheral neuropathy compared with the control group who use the western medicine alone,or in combination with certain kinds of differences in the efficacy of western medicine is statistically significant.(6)According to the different time of each treatment,classification all the researchers to 8 subgroup,combined the Meta-analysis result of 8 subgroup,the MD[95%CI] of each subgroup were 6.00[2.03,17.73]?2.87[1.76,4.68]?4.84[3.74,6.27]?3.81[1.86,7.80]?4.62[2.99,7.15]?2.70[1.34,5.45] ?2.83[1.16,6.89]?8.67[2.28,32.90],it can be concluded that the result of Buyang Huanwu Decoction combined with Western medicine treatment of diabetic peripheral neuropathy is statistically significant even the time of the treatment is different.Conclusion:The effect of BuYang Huan Wu decoction conbine with western medicine in treating DPN is better than only use western medicine,and is nothing to do with what kind of western medicine it combine with,as well as the time the treatment last. |
