Theraqeutic Effects Of Huaiqihuang On Children Henoch-schonlein Purpura Rash Recurrence And Kidney Damage

Objectives: To observe the therapeutic effect of Huaiqihuang Granule on HSP rash recurrence and renal damage.Methods: 1?The study object and groups:158 children with HSP who were treated in nephrology department of our hospital from June 2014 to October 2015, were randomly divided into the Huaiqihuang intervention group(80 cases)and control group(78 cases). Two groups used conventional medical treatment and Huaiqihuang intervention group on the basis of medical treatment plus with Huaiqihuang granules. 2?Observation target : In the early morning and every week detection urine analysis.And follow-up of two groups with rash recurrence,joint pain, gastrointestinal symptoms and kidney damage. At the end of the follow-up of 6 months, or an immediate end of the follow-up when kidney damage appeared in the process.In the next day in the hospital and the end of the follow-up.Detected the level of serum Ig A and Ig G with the method of scattering turbidimetric,and the level of serum IL-2?IL-8 and TNF-? with the method of chemiluminescence.Result: 1?Huai qi huang intervention group compared with control group in various clinical symptoms remission time, found that the relief time of rash[(5.29±3.73)dvs.(4.90±2.55)d,P=0.44],joint symptoms [(2.16±1.15) d vs.(2.32±2.15) d,P=0.64],digestive tract symptom [(3.49±4.27) d vs.(5.28±5.33)d,P=0.13], P>0.05,the difference was no statistically significant. 2?Huai qi huang intervention group compared with control group in the acute serum Ig A and Ig G:Ig A[(2.60±6.01)g/Lvs.(1.93±9.64) g/L], Ig G[(10.39±2.68) g/Lvs.(10.52±5.58)g/L],P>0.05,the difference was no statistically significant. In the remission time the levels of the serum Ig A and Ig G between the two groupsIg A[(1.08±0.31)g/Lvs.(1.58±0.46)g/L,P<0.05],Ig G[(10.01±2.39) g/Lvs.(9.52±1.87) g/L],there is a decrease in the intervention group than control group in the level of serum Ig A,but the level of serum Ig G is not.The level of serum Ig A and Ig G in the intervention group between different time Ig A [(2.60±6.01) g/Lvs.(1.08±0.31) g/L,P<0.05],Ig G [(10.39±2.68) g/Lvs.(10.01±2.39) g/L],there was a decrease of the level of the serum Ig A of the intervention group compared with the acute time(P<0.05), but the level of serum Ig G is not. The level of serum Ig A and Ig G in the control group between different time Ig A[(1.93±9.64) g/L vs.(1.58±0.46)g/L, P < 0.05],Ig G[(10.52±3.58) g/Lvs.(9.52±1.87) g/L],there was a decrease of the level of the serum Ig A of the control group compared with the acute stage(P<0.05), but the level of serum Ig G is not. 3?There were no statistically significant difference of the levels of the serum IL-2?IL-8 and TNF-? in Huai qi huang intervention group compared with control groupin the acute time IL-2 [(632.26±460.65) u/ml vs.(729.57±312.22) u/ml],IL-8 [(940.12±1165.98)pg/mlvs.(1137.9±1215.0) pg/ml],TNF-?[(31.80±146.02)pg/ml vs.(41.29±116.54) pg/ml]. In the remission time the levels of the serum IL-2,IL-8 and TNF-? between the two groups IL-2 [(1223±293.19) u/ml vs.(1087±243.74) u/ml],IL-8[(27.67±13.08)pg/mlvs.(48.67±21.01)pg/ml,P < 0.05],TNF-?[(10.49±5.01) pg/mlvs.(12.55±4.22) pg/ml, P < 0.05], the level of the serum IL-2 of the intervention group was higher than the control group,while the levels of IL-8 and TNF-? were lower.The level of serum IL-2?IL-8 and TNF-? in the intervention group between diffent time IL-2[(632.26±460.65) u/ml vs.(1223±293.19) u/ml, P <0.05].IL-8 [(940.12± 1165.98) pg/ml vs.(27.67±13.08) pg/ml, P < 0.05] ?TNF-?[(31.80±146.02) pg/ml vs.(10.49±5.01) pg/ml, P<0.05], the level of the serum IL-2 in the remission time was higher than the acute time,while the levels of IL-8 and TNF-? were lower. The level of serum IL-2?IL-8 and TNF-? in the control group between different time IL-2[(729.57±312.22) u/ml vs.(1087±243.74) u/ml, P< 0.05],IL-8 [(1137.9± 1215.0) pg/ml vs.(48.67±21.01) pg/ml, P <0.05],TNF-?[(41.29±116.54) pg/ml vs.(12.55±4.22) pg/ml,P<0.05], the level of the serum IL-2 in the remission time was higher than the acute time,while the levels of IL-8 and TNF-? were lower.4?In the process of follow-up Huai qi huang intervention group compared with control group rash recurrence was reduced, the huai qi huang intervention group(8 cases), the control group(18 cases,) P< 0.05, the difference was statistically significant.Renal damage appeared in the process of follow-up(huai qi huang intervention group 9 cases, control group 20 cases),huai qi huang intervention group renal damage relatively less than control group, P<0.05, the difference was statistically significant.But, Huai qi huang intervention group on the digestive tract symptoms, joint systomes recurred treatment effect is not obvious, the difference was statistically significant.Conclusions: 1?Huaiqihuang granules can reduce the HSP, rash recurrence and kidney damage but did not significantly reduce the HSP joint swelling and pain and gastrointestinal symptoms. 2?Huai qi huang granules may reduce the rash recurrence and kidney damage by adjusting the immune dysfunction of the HSP children.
