Research On TCM Syndromes Of HPAG Before And After Anti-Hp Treatment

Objective: By collecting basic information of patients with Hp associated gastritis and TCM clinical symptom scores,distribution of tongue manifestation,Hp clearance rate with Hp-positive patients before and after standard anti-Hp triple therapy,to discuss the mechanism of changes of TCM syndromes after anti-Hp treatment and to support the tutor's viewpoint with statistical analysis;also to provide some new clinical references for TCM research on Hp associated gastritis.Methods:To collect basic information of outpatients and inpatients with Hp associated gastritis in the First Affiliated Hospital of Guangxi University of Chinese Medicine by carrying out an Epidemiological investigation and fabricating a quantization table.To analyse the relationship between TCM Syndromes and both the endoscopic mucosa and Hp infection by determining TCM Syndrome Types based on clinical data.By collecting TCM clinical symptom scores,distribution of tongue manifestation,Hp clearance rate with Hp-positive patients before and after standard anti-Hp triple therapy,to discuss the mechanism of TCM syndromes' changes with Establishment of a database for statistical analysis.Results:(1)Comparing Each of syndromes and the Hp infection of each syndrome : Splenogastric hygropyrexia syndrome>mixed cold and heat syndrome>spleen and stomach syndromes>pleen deficiency syndrome.(2)Comparing the endoscopic mucosa of each syndrome,the difference was statistically significant(P<0.01).Splenogastric hygropyrexia,mixed cold and heat syndrome mainly expressed congestion and edema,erosion,erythema;spleen and stomach,spleen deficiency syndrome mainly expressed red and white,nodular uplift,granular thickening.(3)Comparing the clinical symptom scores before and after anti-Hp therapy,Splenogastric hygropyrexia syndrome:Comparing the scores of epigastric pain,abdominal fullness,dry and bitter mouth,acid reflux,belching before and after therapy,the difference was statistically significant(P<0.05).Comparing the scores of Poor appetite,body weight drowsiness,poor stool,the difference was not statistically significant(P>0.05).mixed cold and heat syndrome:Comparing the scores of epigastric pain,abdominal fullness,dry and bitter mouth,acid reflux,chills,the difference was statistically significant(P<0.05).Comparing the scores of Poor appetite,belching,poor stool,the difference was not statistically significant(P>0.05).Comparing the scores of belching,Poor appetite,loose stool,the difference was not statistically significant(P>0.05).spleen and stomach syndrome:Comparing the scores of epigastric pain,abdominal fullness,acid reflux,pale mouth,loose stool,the difference was statistically significant(P<0.05).Comparing the scores of Belching,fatigue,Poor appetite,the difference was not statistically significant(P>0.05).spleen deficiency syndrome:Comparing the scores of Epigastric pain,acid reflux,belching,fatigue,vomiting water,cold limbs,loose stool,the difference was statistically significant(P<0.05).Comparing the scores of abdominal fullness,Poor appetite,the difference was not statistically significant(P>0.05).(4)Comparing the distribution of furred tongue before and after anti-Hp therapy,the difference was statistically significant(P<0.01).Before treatment,yellow greasy fur> white greasy fur = thin yellow fur> white fur> thin white fur.After treatment,thin white fur > white fur> thin yellow fur = white greasy fur>yellow greasy fur.(5)Comparing the Hp clearance rate before and after treatment,the difference was statistically significant(P<0.05).Conclusion:(1)There existed a certain correlation between TCM Syndromes and both the endoscopic mucosa and Hp infection of patients with chronic gastritis.(2)The TCM clinical symptoms were not so obvious improved,even worse.anti-Hp therapy played a role of clearing heat and dampness,which was not taking into account the spleen and stomach,Even damaged the spleen yang.(3)It was proved that the furred tongue has a significant change before and after anti-Hp treatment,witch was showed the conversion between sufficient and deficiency Syndrome,as well as heat and cold.(4)By carrying out this study witch based on the TCM theory of “spleen and stomach,sufficient and deficiency”,to discuss the mechanism of TCM syndromes' changes and support the teacher's viewpoint from the TCM theory of “the bitter cold medicine spoil one's appetite,” “sufficient and deficiency,cold and heat”.
