| Objective: investigate on the malnutrition of Influence by using the method of reinforcing spleen and kidney to improve the CKD3 phase of chronic kidney disease with kidney qi deficiency.Methods: 60 cases of CKD3 complicated with malnutrition patients in accordance with the inclusion criteria and exclusion criteria in our Hospital of the department of internal medicine were selected from Dec 2013 to Aug 2015,and were randomly divided into treatment group and control group according to the proportion of 1:1.The control group were treated with conventional western medicine including low salt,low fat,high quality protein diet,lower blood pressure,lipid,redress renal anemia,regulating the balance of calcium and phosphorus,keeping water,electrolyte and acid-base equilibrium balance..The treatment of control group is common western medicine treatment and Yishen Jianpi Decoction,which based on the method of strengthening spleen and tonifying kidney,from the TCM therapy of national chronic renal failure non-hemodialysis and correlated therapy The decoction includes Huang Qi?Dang Shen?Bai Zhu?Tu Si Zi?Fuling?Shou Wu?Huai Shan.Combined with the guangxi local patients clinical characteristics of dampness,blood stasis and humid,in the add and subtract.The dampness-people add Fabanxia?Yi Yi Ren,etc;the blood-stasis-people add San Qi?Dan She,etc;The humid-people add Mao Xu Cao?Shu Da Huang,etc.Two groups of treatment time is 3 months,Monitor the Index includes(SGA score,PA,ALB,TC,TF,Symptom)on the patients in the two groups before and after 6weeks,12 weekstreatment,And evaluation it.Results:?SGA Grade: treatment group SGA Grade,include SGA-A 12 cases,SGA-B 17 cases,SGA-C 1 cases before treatment,after treatment was SGA-A 16 cases,SGA-B 14 cases,SGA-C 0 cases,P?0.05,with statistical significance;Control group SGA Grade include SGA-A 11 cases,SGA-B 17 cases,SGA-C 2cases before treatment,after treatment was SGA-A 12 cases,SGA-B 16 cases,SGA-C 2cases,P?0.05,no statistical significance.?PA: the treatment group treats in the former average PA was 119±15.56mg/L,150 ±14.62mg/L after treatment was statistically significant(P ? 0.05),showed kidney tonifying spleen method can increase the PA;Control group PA average was 121±13.48mg/L before treatment,after treatment was 125±14.76 mg/L,no statistical significance(P?0.05).?ALB: the treatment group treats in the former average ALB was 30±5.31g/L,38±4.27g/L after treatment was statistically significant(P?0.05),showed kidney tonifying spleen method can increase the ALB;Control group ALB average was 32±4.61g/L before treatment,after treatment was 29±3.54g/L,no statistical significance(P?0.05)?TC:Treatment group treats in the former TC average was 2.9±0.86mmol/L,3.2±0.57 mmol/L after treatment,P ? 0.05 with statistical significance;Control group TC average was 2.7±0.79mmol/L before treatment,after treatment was 2.8±1.06mmol/L no statistical significance(P?0.05).?TF:the treatment group treats in the former average TF was 119±15.56mg/L,200±16.86mg/dl after treatment was statistically significant(P ? 0.05),showed kidney tonifying spleen method can increase the TF.Control group TF average was 190±10.79mg/dl before treatment,after treatment was 185±9.56 mg/dl,no statistical significance(P?0.05).?Symptom effects: the treatment group effective rate was 83.3%,the clinical alleviate 9cases had marked effect,16 cases of effective,4 cases of stability,1 cases of nullity.In the control group,the effective rate was 56.7%,7 cases had marked effect,10 case effectively,9 cases of stability,4 cases of nullity.There are significant difference(P?0.05)between the treatment group effective and the control group.That means the clinical effects of treatment group is better than the control group.Conclusion: ?Using the way Yishen Jianpi Decoction treats the CKD3 patients can raise PA,ALB,TC,TF,SGA Score,Symptom points to improvie malnutrition status.?Using the way of yishen Jianpi Decoction treats the CKD3 patients canimprove clinical symptoms and the quality of life. |
