Objective: Periodontitis is a chronic infectious disease of the periodontal tissue.The development of periodontitis is caused by the interaction of periodontal pathgens and the immune response of the body.T helper cell 17(Th17),regulation T cells(Treg)and related cytokines play an important role in the pathogenesis of chronic periodontitis.In this study,we examined the levels of Interleukin-17(IL-17),Interleukin-23(IL-23),Interleukin-10(IL-10),transforming growth factor-?(TGF-?)in gingival crevicular fluid(GCF)and serum of 40 chronic periodontitis patients of different severity with the method of enzyme-linked immunosorbent assay(ELISA).The relationship between IL-17,IL-23,IL-10,TGF-? and the clinical indicators of periodontitis was analyzed,to evaluate the role of Th17 and Treg in periodontitis pathogenesis and the immunological basis for the onset and development of chronic periodontitis.Methods: The periodontitis patients were selected from the Department of stomatology of the Dental school of Hebei Medical University.On the basis of their oral condition,patients were divided into 4 groups with 10 patients in the normal group,10 patients in the group of gingivitis,9 patients in the group of slight chronic periodontitis and 11 patients in the group of moderate-severe chronic periodontitis.The periodontal conditions of every tooth were recorded in detail including the plaque index(PLI),the probing depth(PD),the attachment loss(AL),the bleeding index(BI),and so on.And the number of missing teeth except the third molar was recorded.Meanwhile everyone took an oral pantomography.1 Detection of the levels of IL-17,IL-23,IL-10,TGF-? in GCF6 teeth(16,11,26,36,31 and 46)were chosen as the index teeth for each subject.Absorbent paper points were used to collect the GCF from each tooth,and then the mixed GCF was used to detect the levels of IL-17,IL-23,IL-10,TGF-? by ELISA.Then we analyzed the relationship between IL-17,IL-23,IL-10,TGF-? and the clinical indicators of different groups.2 Detection of the levels of IL-17,IL-23,IL-10,TGF-? in serum6 mL peripheral venous blood was collected from each patient early in the morning with an empty stomach.2 mL peripheral venous blood with heparin anticoagulation was used to examine the leukocyte count(include polymorphonuclear,lymphocyte,eosinophils and basophilic granulocyte).The supernatant of the remaining 4 mL peripheral venous blood was used to detect the levels of IL-17,IL-23,IL-10,TGF-? in serum by ELISA.Then we analyzed the relationship between IL-17,IL-23,IL-10,TGF-? in serum and the clinical indicators of different groups.Results:1 The change of the volume of GCFThe volume of GCF in patients of the B,C,D group were significantly higher than that in the A group(P<0.05);the volume of GCF in the C,D group were significantly higher than that in the B group(P<0.05);the volume of GCF in the D group were significantly higher than that in the C group(P<0.05).And there was a positive correlation between the volume of GCF and PD of different groups(r=0.889,P<0.01),as well as AL of different groups(r=0.843,P<0.01).2 The change of IL-17,IL-23,IL-10,TGF-? in GCF and serum2.1 The concentration of IL-17 in GCF and serum in the B,C,D group were significantly higher than that in the A group(P<0.05);the concentration of IL-17 in GCF and serum in the C,D group were significantly higher than that in the B group(P<0.05);the concentration of IL-17 in GCF and serum in the D group were significantly higher than that in the C group(P<0.05).A positive correlation was found between the concentration of IL-17 in GCF and PD,AL(r=0.726,P<0.01;r=0.796,P<0.01),as well as between the concentration of IL-17 in serum and PD,AL(r=0.856,P<0.01;r=0.818,P<0.01).2.2 The concentration of IL-23 in GCF and serum in the B,C,D group were significantly higher than that in the A group(P<0.05);the concentration of IL-23 in GCF and serum in the C,D group were significantly higher than that in the B group(P<0.05);the concentration of IL-23 in GCF and serum in the D group were significantly higher than that in the C group(P<0.05).A positive correlation was found between the concentration of IL-23 in GCF and PD,AL(r=0.830,P<0.01,r=0.863,P<0.01),as well as between the concentration of IL-23 in serum and PD,AL(r=0.855,P<0.01;r=0.859,P<0.01).2.3 A positive correlation was found between the concentration of IL-17 and the concentration of IL-23 in GCF(r=0.777,P<0.01),as well as concentration of IL-17 and the concentration of IL-23 in serum(r=0.743,P<0.01).2.4 The concentration of IL-10 in GCF in the B,C group were significantly higher than that in the A group(P<0.05);the concentration of IL-10 in GCF in the C group were significantly higher than that in the B group(P<0.05);the concentration of IL-10 in GCF in the D group were significantly lower than that in the A,B,C group(P<0.05).A negative correlation was found between the concentration of IL-10 in GCF and AL(r=-0.324,P<0.05).In serum,there was no statistically significant difference in concentration of IL-10 among the four groups(P>0.05).2.5 The concentration of TGF-? in GCF in the B,C group were significantly higher than that in the A group(P<0.05);the concentration of TGF-? in GCF in the C group were significantly higher than that in the B group(P<0.05);the concentration of TGF-? in GCF in the D group were significantly lower than that in the A,B,C group(P<0.05).In serum,there was no statistically significant difference in concentration of TGF-? among the four groups(P>0.05).2.6 A positive correlation was found between the concentration of IL-10 and the concentration of TGF-? in GCF(r=0.794,P<0.01).2.7 The concentration of IL-17,IL-23,IL-10,TGF-? in GCF of all groups was significantly higher than that in serum,respectively(P<0.05).Conclusions:1 The concentration of IL-17,IL-23 in GCF and serum in groups of gingivitis,slight chronic periodontitis and moderate-severe chronic periodontitis were higher than that in normal group.With the increased PD and AL the concentration of IL-17,IL-23 raised.It can be concluded that IL-17,IL-23 were closely related to the severity of chronic periodontitis.IL-17,IL-23 therefore could reflect the degree of periodontal tissue destruction.2 There was a positive correlation between the concentration of IL-17 and IL-23 in GCF and serum,respectively.Suggesting there was an IL-23/IL-17 axis in the development of chronic periodontitis.3 The concentration of IL-10,TGF-? in GCF in groups of gingivitis and slight chronic periodontitis were higher than that in normal group,and the concentration of IL-10,TGF-? in GCF in groups of moderate-severe chronic periodontitis were lower than that in normal group.In serum,there was no statistically significant difference in concentration of IL-10 and TGF-? among the four groups.It suggests that Treg related cytokines played an important role in local immune responses at the periodontal tissue of chronic periodontitis.4 With the increased severity degree of chronic periodontitis the expression of IL-17,IL-23 increased.IL-10,TGF-? expression increased in gingivitis and the early stage of chronic periodontitis,and it decreased in the late stage of chronic periodontitis.It suggests that the imbalance of Th17,Treg and related cytokines appeared in development of chronic periodontitis.
