Purpose: With the general improvement in people's quality of life and the change of diet in recent years,the incidence of acute pancreatitis increased significantly,and hyperlipidemia-induced acute pancreatitis was significantly increased too.Hyperlipidemia has become the most frequent cause of acute pancreatitis except the biliary tract disease and alcohol.We call this kind of pancreatitis hyperlipidemic acute pancreatitis,which causes people's much attention.Hyperlipidemia is also a component of metabolic syndrome.metabolic syndrome is a disease with obesity,hyperglycemia,hypertension,dyslipidemia,and so on.This risk factors coming together can lead to the initiation and progression of type 2 diabetes and cardiovascular disease.Metabolic syndrome is often coexist with some diseases such as fatty liver,cholesterol gallstone disease,asthma and even some carcinomas.The incidence of metabolic syndrome also increased obviously in recent years,and metabolic syndrome has become a new kind of chronic disease and public health problem.We analyze the clinical characteristics of the hyperlipidemic acute pancreatitis with the clinical information collected in our article to discuss the relationship between hyperlipidemic acute pancreatitis and metabolic syndrome.Method: O ur study collect the patients who experienced the attack of acute pancreatitis and were admitted in our hospital between January 2011 and August 2015.Patients should have comprehensive datum.Then we choose the hyperlipidemic acute pancreatitis in this patients.The diagnostic criteria for acute pancreatitis and hyperlipidemic acute pancreatitis is on the basis of the diagnosis and treatment of acute pancreatitis of 2014 formulated by the pancreatic surgery group in the surgery branch of Chinese medical association.The clinical information of hyperlipidemic acute pancreatitis include age,gender,vital signs,laboratory examination,imaging examination,hospitalization days,treatment and prognosis,and the medical history(diabetes,hypertension,fatty liver,etc.)We assess the severity of the hyperlipidemic acute pancreatitis by the diagnosis and treatment of acute pancreatitis of 2014 and describe the clinical characteristics of them.Then we analyze if this patients fulfill the metabolic syndrome diagnostic criteria on the basis of the metabolic syndrome diagnostic criteria suggested by the C hinese Diabetes Society in 2004.Result: A total of 114 patients were included in our study.17 hyperlipidemic acute pancreatitis is diagnosed(15 males and 2 females,the sex ratio is 7.5:1),which account for 14.9% of the acute pancreatitis.Age ranges from 28 to 56 years old,with an average age 39.2 years old.BMI is from 20.72 to 35.43 kg/m2,with an average 27.72 kg/m2.We have 13 mild acute pancreatitis,3 moderately severe acute pancreatitis and 1 severe acute pancreatitis in this 17 cases.16 cases were treated with conservative treatment and discharged from hospital early,while 1 cases were treated with surgical treatment who was diagnosed severe acute pancreatitis and had a long duration of hospital stays(83 days).The average duration of hospital stays is 16.1 days.All of this cases were cured.We found that 16 cases(94.1%)are overweight and(or)obesity,15 cases(88.3%)have hyperglycemia,6 cases(47.0%)have hypertension,17 cases(100%)have dyslipidemia,10 cases(58.8%)have chylemia,14 cases(82.3%)have the imaging of fatty liver and(or)have a medical history of fatty liver.We implemented the parameter of the definition of metabolic syndrome on our patients and found that 17(100%)of patients with hyperlipidemic acute pancreatitis fulfilled the criteria of metabolic syndrome suggested by the Chinese Diabetes Society in 2004.Conclusion: 1.We first propose that the hyperlipidemic acute pancreatitis patients have the features of metabolic syndrome and can be a part of the metabolic syndrome.2.When hyperlipidemic acute pancreatitis attacks a person,we clinician should not only treat the pancreatitis,but also take measures for the treatment of metabolic syndrome as soon as possible to slow down the progress of the pancreatitis in time and improve the prognosis.3.The early intervention of the metabolic syndrome may reduce the incidence of the hyperlipidemic acute pancreatitis,decrease the times of attack and reduce the medical burden.
