Clinical Outcomes Of Arthroscopic Subacromial Decompression Versus Arthroscopic Subacromial Debridement For Curved Acromion In Subacromial Impingement Syndrome

Objective : An retrospective analysis of the difference between arthroscopic subacromial decompression and arthroscopic subacromial debridement for curved acromion in subacromial impingement syndrome,in order to find the better treatment.Materials and Methods: From 2013 to 2014,57 patients with subacromial impingement syndrome who had been diagnosed a curved acromion from the radiography in the hospital have been involved.Every patient finished the follow-up of mean 1 year.A group has 27 patients under arthroscopic subacromial depression,which contains 20 men and 12 women with a mean age of 52 years(39 to 68).B group contains 25 patients under arthroscopic subacromial debridement,including 13 men and 12 women with a mean age of 54 years(42 to 70).Before surgery was considered,all patients followed a protocol of conservative treatment consisting of anesthetic injections into the subacromial space,combined with NSAIDs,physiotherapy and a period of exercise therapy.When conservative therapy failed,arthroscopy of the shoulder was performed by an experienced shoulder surgeon.Date measures employed at pre-operation and post-operation are The UCLA Shoulder Rating Scale and Visual Analogue Scale(VAS).Results: Postoperative symptoms and function improved significantly.After 1 year.Good or excellent results were found in 88% in the arthroscopic subacromial depression group 88% and in 84% in the arthroscopic subacromial debridement group.The arthroscopic subacromial debridement group had more early symptoms of mitigation.Conclusions:Two surgical methods can significantly improve symptoms and function in patients with curved acromion.In the treatment of patients with a curved acromion,arthroscopic subacromial debridement has advantage.
