Research On Therapeutic Touch To The Enhancement Of The Comfort Of Patients With Trans-radial Coronary Interventions

Objective:To study patients' comfort level and limb pain post Trans-radial coronary intervention(TCI).To analyze the factors that affect the comfort of patients and limb pain post TCI.To explore the effectiveness of therapeutic touch intervention on enhancing patients' comfort and reducing limb pain post TCI.Method:106 subjects underwent TCI were randomly allocated into control group of 55 cases and study group of 51 Cases. The effective cases were 103, 54 cases of control group, 49 cases of study group. Patients in control group received routine post-op nursing, and at 2 hours and 4 hours post respectively release the radial artery compressor scale semi-circle, at 6 hours post completely release pressure. On the basis of the same treatment, patients in study group received therapeutic touch intervention on affected limb after releasing the radial artery compressor at 2 hours and 4 hours post. Evaluate patients' limb pain scores at different times, comfort, limb hematomas and hemorrhage during the study. Research tools include general information questionnaire, Numerical Rating Scale(NRS), General Comfort Questionnaire(GCQ). Descriptive analysis, t test, rank sum test, ?2 test, Spearman rank correlation analysis was used for data analysis.Result:1. Different degree of limb pain existed in patients post TCI. At 2 hours post median score of NRS was 5 points, the highest score was 7 points, the lowest score was 1 point, and 59.22% at middle level. At 4 hours post median score of NRS was 6points, the highest score was 9 points, the lowest score was 3 points, and 47.57% at middle level.2. In the study group patients' limb pain scores rose at 2 hours post immediately and at 4 hours post immediately. Two independent samples t-test shows the difference is also statistically significant(P<0.01). In the study group patients' limb pain scores rose at 2 hours post intervention and at 4 hours post intervention.Two independent samples t-test shows the difference is also statistically significant(P<0.01). In the control group patients' limb pain scores rose at 2 hours post immediately and at 4 hours post immediately. Two independent samples t-test shows the difference is also statistically significant(P<0.01). In the control group patients' limb pain scores rose at 2 hours post intervention and at 4 hours post intervention.Two independent samples t-test shows the difference is also statistically significant(P<0.01).3. In the study group, patients' limb pain scores at 2 hours and 4 hours post intervention were lower than pre intervention(P<0.01). In the control group, patients' limb pain scores at 2 hours and 4 hours post relieving were lower than before the release of oppression(P<0.01). At 2 hours post, the study group had better pain reduction than control group, two independent samples t-test shows the difference is also statistically significant(P<0.01). At 4 hours post, the study group had better pain reduction than control group, two independent samples t-test shows the difference is also statistically significant(P<0.01).4. Post TCI, mean distribution of overall comfort score of study group was 66.21±6.22,and 65.72±5.75 for control group. Study group had higher score in physical dimension and emotional dimension than control group and the rank-sum test shows the difference is statistically significant(P values are less than 0.01).5. In the study group and control group, the score of item "I can not stand the pain" was the lowest. Study group had higher score in four items "I can not stand the pain ", "I don't want to move ", "I'm not feeling well ", " I feel very tired " in physical dimension and five items " I feel confident", " I'm afraid of what's going to happen ", "I don't have a sense of belonging here ", " I feel calm ", " I'm feeling down" in emotional dimension, two independent samples t-test shows the difference is statistically significant(P values are less than 0.01). Patients' limb pain scores at 2hours post immediately and at 4 hours post immediately were negatively correlated with the overall comfort and the four dimensions.6. Post TCI, hemorrhage occurred in 2 cases in study group and 1 case in control group. Test shows no statistically significant difference between twogroups(P=0.514). There was no hematoma occurrence in both groups. Therefore Test shows no significant difference in hematoma occurrence between two groups(P<0.01).Conclusion:1. Different degree of limb pain existed in patients post TCI, most of them were in moderate pain level, and increased with the increase of the time of radial artery compressor oppression. This situation needs to be intervened.2. Therapeutic Touch intervention can better control patients' pain on affected limb post TCI, and improve patients' comfort.3. Therapeutic Touch intervention is relatively safe on post TCI patient and it does not increase the risk of bleeding and hematoma.
