Objective: Anterior knee pain, which always secondary to patella labilization and overload of the subchondral bone,is one of the most common musculoskeletal complaints of young patients. We notice that some of the patients with normal femoral trochlear, medial and lateral patellofemoral ligaments possessed negative patella configuration in our clinical practice.Impingements of the lateral patellar facet and the femoral trochlear occurred as knee flexion progresses.A specific term to identify the pathologic process would be “lateral patellar impingement syndrome”.We hire ambispective cohort study to explore the curative effect of arthroscopic lateral patelloplasty and therapeutic strategies for early lateral patellar compression syndrome.Methods:1 Thirty-five patients we treated between June 2007 to June 2015,performed arthroscopic lateral patelloplasty, met with our inclusion criteria.Among them included 14 men and 21 women, with a mean age at surgery of 26.2 years(12~50 years)and a mean duration time of 19.3 months(1~60 months).2 All operations were performed by the same senior author.3 Evaluations consisted of pre-and post-operative symptoms, physical examinations,radiographs(X-ray at positive lateral of knee joint,Merchant X-ray film,Computed Tomography scan and Magnetic Resonance Imaging)and questionnaires. The main outcome measure was the Lysholm score and the congruence angle, patellar titl angle and patellar lateral shift was measured with the CT scan and Merchant x-ray film for knee joint. Results: The mean followed up period was 41.0 months(10~78 months).Subjectively,6 patients(17.1%)were very satisfied,26(74.3%)satisfied,2(5.7%)were indifferent, and 1(2.9%)were dissatisfied and would not repeat the procedure. There were statistical differences in pre-(80.66±5.51)and postoperative(81.91±6.21)Lysholm scores. The pre-and postoperative congruence angle, patellar titl angle and patellar lateral shift were(+27.71±8.86)° and(+10.80±8.17)°,(11.60±3.44)° and(10.86±3.16)°,(12.29±2.37)mm and(8.77±2.18)mm,respectively.Statistical difference was found in all groups. Conclusions: It is an effective, essential, minimal invasive and simple method to perform arthroscopic lateral patelloplasty on patients who are lateral patellar impingement syndrome,Based on symptomatic treatment.
