The Analysis Of Experience Elements And Clinical Application On The Treatment Of Chronic Hepatitis B Of Professor Lan Qingqiang

Part one: The analysis of Experience elements on the treatment of chronic hepatitis B of Professor Lan Qingqiang.Objective: In order to study and inherit Clinical experience and academic thoughts on treating chronic hepatitis B of Professor Lan Qingqiang,I use data mining methods of statistical analysis,frequency analysis,system cluster and association rule to analyze laws of prescription medications on treating chronic hepatitis B of Lan Qingqiang.Methods:I record the original medical cases of treating chronic hepatitis B with the text.Then input the information and data of medical cases which meet the requirements into Excel,and establish information database.By using statistical software SPSS 19.0,SPSS Modeler 14.1 using for data mining,I employ methods such as frequency analysis,system cluster,Apriori algorithm and so on for data mining.Then discuss the clinical experience on treating chronic hepatitis B of Lan Qingqiang from syndrome types and formulas,summarize and refine laws of prescription medications and academic thoughts on data mining result.Results:(1)The most common syndrome type of chronic hepatitis B is the syndrome of liver-depression and spleen-deficiency,then followed by the syndrome of Yin deficiency of liver and kidney,the syndrome of Yangdeficiency of spleen and kidney,the syndrome of blood stasis,the syndrome of obstruction of damp-heat.(2)Professor Lan,s commonly used Prescription ingredients to treat chronic hepatitis B are 53 kinds of high frequency medicines,such as Astragale,heterophylla,poria,Bupleurum,rhizomacyperi,angelica,RadixPaeoni aalba,atractylisovata,wolfberry,glossy privet fruit,schisandra chinensis and so on.(3)The main drug kinds of Professor Lan using to treat chronic hepatitis B are tonifying deficiency medicine,heat-clearing medicine,energen-regulating medicine,blood-activating and stasis-eliminating compound medicine,diuresis and diffusing dampness medicine,damp-resolving medicine,digestant medicine,diaphoretics medicine,Mind-easing medicine,astringent medicine,phlegm-resolving medicine,inner-warming medicine.(4)Carry on cluster analysis on high frequency medicines,then get three cluster groups.They are mainly for liver depression,spleen deficiency,kidney deficiency,blood stasis,dampness-heat pathological factors and so on.(5)The common herbal pair of Professor Lan using to treat chronic hepatitis B are Bupleurum and RadixPaeoniaalba,angelica and RadixPaeoniaalba,rehmannia glutinosa and cornus officinalis,Astragale schisandra,chinensiswolfberry,glossy privet fruit group and so on.(6)The common methods of Professor Lan using to treat chronic hepatitis B are soothing the liver and regulating the circulation of qi,strengthening spleen and excreting dampness,reinforcing liver and kidney,nourishing andregulating the liver,supplementing Qi and nourishing Yin and so on.Conclusions:(1)The most common syndrome type of chronic hepatitis B is the syndrome of liver-depression,and other sort in turn are Yin deficiency of liver and kidney,the syndrome of Yang deficiency of spleen and kidney,the syndrome of blood stasis,the syndrome of obstruction of damp-heat.(2)chronic hepatitis B,s disease location is mainly involved with to the liver,spleen and kidney,the most common pathogenic and pathology characteristics of chronic hepatitis B is liver-stagnation and spleen-deficiency.(3)The chronic hepatitis B,s pathological nature has deficiency and excess.The deficiency nature includes spleen deficiency,kidney deficiency,Qi deficiency,Qi and Yin deficiency,liver blood deficiency,and the excess nature includes Liver depression and Qi stagnation,blood stasis,dampness and so on.(4)Professor Lan frequently employs 53 herbs to treat chronic hepatitis B,and the major component kind is tonic herb.When prescribing,he not only emphasis on some ways,but also take attention to many aspects at the same time.Commonly he uses 19 herb pairs and 14 herb groups.(5)Professor Lan apply the clinical thinking of treating liver and spleen together,nourishing kidney for enduring illness,nourishing the liver,protecting vital qi as the basis through the whole period of treating chronic hepatitis B.(6)Data mining results prove that there are rules to follow of Professor Lan,s prescription medications for treating chronic hepatitis B,and reveal the pathogenesis and pathology characteristics of chronic hepatitis B,provide clues for refining the academic thinking of Professor Lan at the same time.Part two:The clinical observation and research of the empirical formula of Professor Lan on treating chronic hepatitis B.Objective: To validate the empirical formula,stherapeutic effect on treating the type of hepatic stagnation and spleen deficiency chronic hepatitis B.And then evaluate the results of data mining.Methods: Firstly,collect 40 cases of hepatic stagnation and spleen deficiency chronic hepatitis B patient at the outpatient department.Then divide the patients into two groups with random number table method,the treatment group and the control group,20 cases in each group.Treat the control group with entecavir and treat the treatment group with entecavir and the empirical formula.Evaluate the clinical efficacy by observing the change of the liver function,HBsAg,HBeAg,HBV DNA before and after the treatment.Results:Results from the blood serum,the control group and the treatment group,sALT,AST level after treatment reduced significantly(P<0.05)compared with before treatment;the control group,s HBsAg,HBeAg Seroconversion and HBV DNA negative conversion ratio after treatment are 11(55%),7(35%),8(40%)respectively,and the treatment group,sHBsAg,HBeAg Seroconversion and HBV DNA negative conversion ratio after treatment are 12(60%)?11(55%)?12(60%)respectively;the control group,stotal effective rate is 60% and the treatment group,stotal effective rate is 75%,there is no difference between the treatment group and the control group of the result(P>0.05).Results from the TCM(Traditional Chinese Medicine)accumulated symptom scores,the control group,ssymptom scores did not reduce significantly(P>0.05)after treatment,the treatment group,ssymptom scores reduced significantly(P<0.05)after treatment;the control group,stotal effective rate is 45% and the treatment group,stotal effective rate is 90%,the treatment group,stotal effective rate is significantly(P<0.05)higher than the control group,stotal effective rate.Conclusions: The Empirical formula of Professor Lan is effective for treating chronic hepatitis B.The data mining result accords with the practice of clinical condition,and data mining is an effective approach for the inheritance of famous and veteran experts in Traditional Chinese Medicine.
