Summarizing Professor Jingri Xie’s Experience Of Syndrome Differentiation And Treatment In Treating Chronic Hepatitis B Based On Data Mining

Objective: To conduct data mining and analysis Professor Jingri Xie’s clinical cases of Chronic hepatitis B by using Traditional Chinese Medicine(TCM)inheritance assistant platform,and to summarize Professor Jingri Xie’s experience of syndrome differentiation and medication in treating Chronic Hepatitis B.Methods: From January 2016 to January 2020,a total of 160 patients,conformed to Professor Jingri Xie’s outpatient department from the First Affiliated Hospital of Heilongjiang University of Chinese Medicine,who met the western medical diagnostic criteria for Chronic Hepatitis B and other criteria of this study were collected.Extracting the information of the first diagnosis of the patients,standardizing the technical terms of the clinical cases,then completely and accurately entry it into TCM inheritance auxiliary platform(V2.5).Selecting the corresponding function of "platform" to analyzes cases of primary survey data.Extracting data from Professor Jingri Xie’s clinical cases in the treatment of Chronic Hepatitis B,which including common symptoms,core drugs and the association rules among TCM symptoms,TCM syndrome types and traditional Chinese medicines,etc.And forming an intuitive network display chart,or exporting the results of data analysis to Excel 2019 and drawing the corresponding table,bar chart or pie chart.Results:1.In this study,there were 160 cases of Chronic Hepatitis B,including97 male patients(60.63%)and 63 female patients(39.37%),among which 54patients(33.75%)were aged between 36 and 45 years old;2.There were a total of 118 TCM symptoms were involved in this study.The top 10 symptoms were: hypochondriac pain(153),costal distension pain(94),pulse and string slipiness(67),yellow-greasy tongue coating(64),red tongue(58),abdominal distension(54),anorexia(50),body fatigue(50),sleep loss(48),tooth marks on the tongue(47);3.This study involved a total of 7 TCM syndrome types,the frequencies were: dampness-heat syndrome of liver and gallbladder(46),liver depression and spleen deficiency syndrome(40),stagnation of liver and spleen deficiency combined with dampness-heat syndrome(24),blood stasis and collaterals obstruction syndrome(19),deficiency of liver and kidney Yin syndrome(14),deficiency of spleen and kidney Yang syndrome(9),disharmony of liver and stomach syndrome(8);4.There were a total of 26 TCM treatments involved in this study,and the top 10 in frequency of use were: Clearing heat and detoxification method(69),Soothing liver and promoting gallbladder method(45),Invigorating spleen and eliminating dampness method(30),Dispersing stagnated liver qi to relieve depression method(24),Soothing liver and strengthening spleen method(24),Promoting blood circulation to remove blood stasis method(16),Promoting qi flow and soothing liver method(15),Nourishing Liver and Kidney Method(14),Nourishing Yin and clearing heat method(14),Dampness-reducing and detoxification method(11);5.This study involved a total of 147 traditional Chinese medicines,and the top 10 in frequency of use were: Radix isatidis(144),Forsythia(143),Radix bupleuri(142),Atractylodes(138),Chicken’s gizzard-membrane(115),Coix seed(113),Tangerine peel(112),Astragalus membranaceus(100),Bergamot(94),Perilla fruit(92);6.In this study,there were a total of 19 kinds of traditional Chinese medicines.The top 7 in frequency of use were: deficiency tonic(581),heat-clearing(581),water-enhancing and humidifying(482),qi-regulating(397),humidifying(214),invigorating blood circulation and removing blood stasis(209),and antidotes(202);7.In terms of traditional Chinese medicines medicinal property,medicinal taste and return meridian,the frequency of medicinal properties were: warm(1250),cold(1186),flat(679),cool(213)and hot(4);The frequency of medicinal tastes were: bitter(1790),Gan(1575),Xin(1249),sour(354),salty(201)and astringent(80);The frequency of return meridians were: spleen meridian(1649),stomach meridian(1420),liver meridian(1374),etc.8.In terms of prescription rule,support degree was set at 80(50.0%)and confidence degree was set at 0.99,148 combinations of 2-6 drugs were obtained,and 116 associated rules were extracted;9.In terms of the new prescription combination,the correlation degree was set as 8 and the punishment degree was set as 2,14 core drug groups of the new prescription were obtained,and 7 groups of new prescription combinations for Chronic Hepatitis B were extracted;10.In terms of drug and symptom rules,appropriate support degrees and confidence degrees were set to obtain the common symptom combinations and commonly used drug combinations of the 7 TCM syndrome types.Conclusion:1.Professor Jingri Xie believes that Chronic Hepatitis B is mainly caused by deficiency of vital qi and invasion of dampness-heat virus;The main pathogenesis are dampness-heat accumulation,stasis of liver collaterals,deficiency of liver,spleen and kidney.Common TCM symptoms are hypochondriac pain,pulse and string slipiness,yellow-greasy tongue coating,abdominal distension,anorexia,body fatigue.The most common TCM syndrome types are dampness-heat syndrome of liver and gallbladder and liver depression and spleen deficiency syndrome.2.In the treatment of Chronic Hepatitis B,Professor Jingri Xie advocated "invigorating the spleen and reinforcing stomach,caring for the root","clearing away heat and detoxification,as well as dehumidification","promoting blood circulation and removing blood stasis,and slowly dredging the liver collaterals";In terms of medication,the drugs are mainly belong to tonify deficiency,clear heat,sexually warm and cold,taste Gan,bitter and Xin,return to spleen,stomach and liver meridian.The commonly used drugs are atractylodes,astragalus membranaceus,radix isatidis,forsythia,radix bupleuri,chicken’s gizzard-membrane,coix seed,tangerine peel,bergamot,perilla fruit,etc.
