Objective:To analyze the clinical efficacy of minimally invasive percutaneous plate osteosynthesis (MIPPO) and otraditional pen reduction and internal fixation (ORIF) combined with proximal humeral locking system (PHILOS) for treatment of proximal humerus Neer?, ? type fractures in elderly patients. Methods:A retrospective analysis of151cases of proximal humerus in elderly March2010to April2013in our hospital clinical data of patients with fractures, a total of68patients with met inclusion and exclusion criteria, both for the proximal humerus elderly Neer?.? type fractures. MIPPO technique which uses a combination PHILOS treatment of proximal humerus elderly Neer?,? type fractures were37cases (MIPPO group). traditional open reduction and internal fixation (ORIF) PHILOS treatment of proximal humerus elderly Neer?,? type fractures31patients (ORIF group). Record incision length, operative time, blood loss, hospital stay, fracture healing time. functional recovery and complications. and analyzed statistically. Results:68patients were followed up for12to36months (mean19.7months), MIPPO group incision length, operative time, blood loss, hospital stay, healing time is more obvious than ORIF group, the difference was statistically significant (P<0.05); after3months.6months. Constant-Murley shoulder score MIPPO group than the ORIF group, the difference was statistically significant (P<0.05):postoperatively December Neer and Constant-Murley shoulder score (CMS), no statistically significant difference (P>0.05). Incidence of postoperative complications MIPPO group than the ORIF group, but no statistically significant difference (P>0.05). Conclusion:MIPPO combined with PHILOS treatment of proximal humerus Neer?. ? type fractures in elderly patients meets the principles of biological fixation, have a slight advantage than traditional open reduction and internal fixation, such as:trauma, smaller incisions, shorter hospital stay, less bleeding, shorter operative time, the advantages of early fracture healing, is the treatment of proximal humerus elderly Neer?,? type fractures effective method.
