Aesthetic Evaluation Of Bordline Case Of Skeletal Class Ⅱ

Objective:The aims of this study including two aspects:1. To study the influence of incisor position on facial profile attactiveness of skeletal class II patient during camouflage orthodontic treatment. 2.To making a Comparison among three different treatment strategies of Skeleton Class II patient.Methods: A Chinese young female with skeletal class II hard and soft tissue was chosen as research object.1. Photoshop software was chosen to change the position of incisors,then those changes were reflected to lip based on accepted ratios of soft tissue to hard tissue changes.Finally 12 different profile pictures were achieved. professional orthodontists and non-professionals were chosen to score for these pictures.2. chose the highest score of part I as effect of camouflage orthodontic treatment. Additionally, the effect of genioplasty combine with orthodontic treatment and orthognathic treatment were simulated by Photoshop software. Finally 6different profile pictures were achieved. professional orthodontists and non-professionals were chosen to score for these pictures.Results:1. The preferable profiles of camouflage orthodontic treatment were those with lower incisor inclination of 90 or 92.5 degrees, and those with FA Points of maxillary incisors 1 or 2mm behind the Goal Anterior Limit Line(GALL line).2.As for chin projection of skeleton class II patient during genioplasty,professional orthodontists and non-professionals chose pogonion advancement of 4mm as the preferable one.In addition, the prolile worsened during closest approach to lower lip margin.3.professional orthodontists and non-professionals reach an agreement on orthognathic treatment got better profile than camouflage orthodontic treatment.4.From the aspects of aesthetics, genioplasty combine with orthodontic treatment is better than camouflage orthodontic treatment alone.However, genioplasty combine with orthodontic treatment is inferior to orthognathic treatment.Conclusion:1. In skeletal class II adult camouflage orthodontic treatment,attractive soft tissue profiles were achieved when the lower incisor and upper incisor were kept in a labial and lingual tipping position respectively,and the ideal lower incisors labial inclination was kept between 90° and 95°.Excessive labial inclination of lower incisors easilyruin a pleasing smiling appearance.2.”The chin should not be advanced beyond the retrusive lower lip in patient with class II skeletal deformity ” this also applies to Chinese adult female.3. This study supported the conclusion that orthognathic treatment got better profile than camouflage orthodontic treatment.4.From the aspects of aesthetics, genioplasty combine with orthodontic treatment is better than camouflage orthodontic treatment alone.However, genioplasty combine with orthodontic treatment is inferior to orthognathic treatment.
