Objective PTH is an important endocrine hormone to regulate bone metabolism.Recombinant human PTH is mainly used in postmenopausal osteoporosis.Alendronate is a classic anti-osteoporotic drug, which has a great effect on postmenopausal osteoporosis. Some researches suggest that PTH and alendronate not only have influence on bone metabolism, but also have the association with the changes of body fat tissue. We will discuss the effects of recombinant human parathyroid hormone and alendronate on body composition in postmenopausal osteoporosis patients. Methods Selecting 64 postmenopausal osteoporosis patients, randomized into two groups(42 patients in PTH group and 22 patients in ALN group). The two group were given treatment with PTH and ALN respectively for 48 weeks, and observed changes of body composition by Dual Energy X-ray Absorptiometry. Results The PTH administration patients had a increase on total body fat mass percentage, upper limbs fat mass, android fat mass, gynoid fat mass and visceral fat mass. The patient treated with ALN had little statistical difference in fat tissue except of total body fat mass percentage and trunk fat mass. However, there was a decline of body lean mass both two groups. Serum PTH level had a positive relation with BMI,body weight, total body fat mass, trunk fat mass, android fat mass, gynoid fat mass and visceral fat mass, and had no relation with body lean mass and bone mineral content. Conclusion The subcutaneous fat tissue and visceral fat tissue of postmenopausal osteoporosis patients were increased after treated with PTH.There was little change of fat tissue after treated with ALN in postmenopausal osteoporosis patients. There was a decrease of lean tissue after treated with PTH or ALN.Serum PTH levels of postmenopausal osteoporosis patients had an association with BMI,body weight and body fat tissue.
