Objective: Introducing the concept of Prime and Fundamental Balanced Scorecard performance evaluation head nurse to build a more reasonable and effective performance appraisal index system of head nurse in three grade hospitals, provide a reference basis for hospital managers to make scientific and efficient performance management.Methods: Under the guidance of Prime and Fundamental Balanced Scorecard, through literature review and semi-structured interview method, preliminary build the head nurse performance appraisal indicators. By the method of two rounds of Delphi Method, the head nurse appraisal indicators shall be drawn up by the preliminary further screening and confirmation, use the analytic hierarchy process(AHP) to determine the index weight. The reliability and representativeness of expert conclusion was examined by positive coefficients of experts, authority coefficients of experts, the coefficients of variation and the concordance coefficient.Results: 1.Under the guidance of Prime and Fundamental Balanced Scorecard, we built the performance appraisal index system of head nurse, composed of two parts, which were Main Performance and Basic Performance, and the former was divided into dominant performance, temporary tasks and short board.2.Dominant performance includes five indicators, as nursing quality, nursing safety, human resources management, communication and coordination, innovation ability, the importance assignment mean of the five indicators is 4.03~4.98, the coefficients of variation is 0.03~0.14. 3.Temporary tasks includes temporary work and the judgment and handling of emergencies, the importance assignment mean are 3.83 and 4.75, the coefficients of variation are 0.19 and 0.10. 4.Short board includes three indicators, people from position matching degree, nursing personnel training and the head nurse’s management level, the importance assignment mean is 4.38~4.63, the coefficients of variation is 0.11~0.17. 5.Basic performance includes administrative and business management, financial and material management, work attitude and responsibility, the importance assignment mean is 4.13 ~ 4.85, the coefficients of variation is 0.07 ~ 0.16. 6.Positive coefficient of two rounds of expert consultation were 95% and 93% respectively(> 70%), the authority of experts coefficient of each round was 0.92 and 0.885(> 0.7), two rounds of expert consultation coordination coefficient was 0.232~0.357, the test of significance were P <0. 001.Conclusion: Built the performance appraisal index system of head nurse which was consisted of 13 level-I indexes and 38 level-II indexes, and determined the connotation and the weight of every index. The results of positive coefficients of experts, authority coefficients of experts, the concordance coefficient show that the results of the expert consultation were reliable. The established system can reflect the functions and responsibilities of the head nurse in three grade hospitals objectively and comprehensively, is beneficial to improve the head nurses’ management and the quality of nursing. The system may be used as a reference for appraising the performance rating of head nurses.
