| ObjectivesTo work out a transitional care instruction handbook for PICC patients,providing PICC patients with continuous guidance, delivering the related information for PICC maintenance,promoting the safety of PICC patients and improving the quality of PICC transitional care.Methods1 In-depth interview was conducted to explore the real needs for transitional care instruction handbook for PICC patients in PICC patients and specialty nurses. Determine the number of interviewer with the principle of information saturation. With Colaizzi method, their major needs were proposed. Preliminary formed the framework of transitional care instruction handbook for PICC patients.2 The transitional care instruction handbook for PICC patients was set up after two questionnaire investigations with Delphi to interview 16 PICC nursing experts. Kendall ’s W cooperative coefficient test were also applied to build the handbook.3 According to the inclusion and exclusion standards, 130 PICCpatients were selected as research subjects, including 63 cases in intervention group and 67 cases in control group by randomly grouping.PICC usual care was conducted in control group and transitional care instruction handbook for PICC patients in intervention group. Through analyzing PICC patients’ self-management ability, satisfaction with PICC,complication occurrences, maintenance compliance and causes of extubation, evaluated the application effects and further improved the content of transitional care instruction handbook for PICC patients ResultsTransitional care instruction handbook for PICC patients compose of 4primary contents, 10 second-level contents, and 43 specific contents. The questionnaire effective recovery rates in the two rounds are 94.1% and100% respectively. The expert authority coefficient in every round is over0.9. The harmonious coefficient is statistically significant after testing(p<0.01). Only two contents’ variation coefficients are more than 25% after two rounds.Effect evaluation of transitional care instruction handbook for PICC patients. The score of PICC patients self-management in intervention group is 161.6±5.7 and that in control group is 141.7±5.0(p<0.01). The total occurrence rate of complications in intervention group is 17.5% that is lower than 34.3% in control group(p<0.05). The score of satisfaction questionnaire in intervention group is 36.06±3.21 and that is 30.84±2.95 incontrol group(p < 0.01). But the two groups’ causes of extubation and maintenance frequencies are no statistically significant difference(p >0.05).ConclusionsTransitional care instruction handbook for PICC patients provides continuous guidance and delivers information about PICC maintenance and self-management for PICC patients. Applying the handbook can improve the self-management ability and satisfaction of PICC patients, and reduce the occurrence rate of catheter related complications.So transitional care instruction handbook for PICC patients can promote the safety of PICC patients and improve the quality of PICC transitional care. |
