Objective :This study aimed to observe the effect of 1,25(OH)2D3 on interleukin 4(IL-4)Ig A nephropathy rats, To investigate the effect of 1,25(OH) 2D3 on the immune regulation of Ig AN in rats, to provide a new way of thinking for the clinical treatment of Ig A nephropathy.Method:(1) Choosing Wister 52 rats, with which 8 rats were choosed in control group(group A) randomly, and the rest of 44 rats were established Ig A nephropathy model by using antigen(bovine gamma globulin) to stimulate the mucosal immune system directly.(2) Removing the dead rats, the successful model of the 32 rats were randomly divided into intervention group(group B) and prednisone group(Group C), 1,25(OH)2D3group(Group D), and prednisone+1,25(OH)2D3 combination therapy group(Group E),the treatment group was given medical intervention, the normal group and the non intervention group were given normal saline orally.(3) The changes were detected in rat urine red blood cell count, 24 hours urine protein, serum creatinine, serum calcium and phosphorus, The levels of IL-4 m RNA in rat spleen lymphocytes were detected by real-time fluorescence quantitative polymerase chain reaction(PCR Real-time). IL-4 protein levels were detected by immunohistochemistry and Western blot in rats.Results:(1)24 hour urinary protein(mg/24h): normal control group(group A) was7.27±0.86, group B(nonintervention group)to49.37±2.96, prednisone group(C group)was 15.26±1.57,group D(1,25(OH)2D3 treatment group) was 21.05±2.06,group E(prednisone +1,25(OH)2D3 combined treatment therapy group) was 10.16±1.39. After treatment, C, D and E were decreased in 24 h group than in B group(P<0.05), and the E group decreased most significantly(P<0.05).(2)Urine red blood cell count(/ul): A group was 5.03±2.55; B group was 328±7.04,C group was 18 ±3.06,D group was 162±5.36, E group was 9±3.15. After treatment, C, D,E group urinary red blood cell count was decreased(P<0.05) compared with the B group,the E group decreased most significantly(P<0.05).(3) Level of blood creatinine of rats(μ mol/L) in each group: group A was42.76±2.42; group B was 45.23±2.01, group C was 45.21±1.52, group D was 43.98±2.36,group E was 43.44±2.42. level of serum calcium of rats(mmol/L) in each group: group A was 2.35±0.22, group B was2.28±0.17, group C was 2.38±0.09, group D was 2.44±0.11,group E was 2.50±0.14. The rats serum phosphorus level(mmol/L): the groups of A,B,C,D and E are respectively 2.85±0.27, 2.90±0.32, 2.86±0.22, 2.89±0.33, 2.80±0.25.Serum creatinine, serum calcium and phosphorus levels showed no significant change,the difference was not statistically significant(P>0.05).(4) the mean density of IL-4:the groups of A,B,C,D and E are respectively0.080±0.030, 0.230±0.013, 0.162±0.021,0.199±0.031, 0.092±0.024.After treatment, the mean density of C,D,E groups were decreased(P<0.05)than B group, and the E group was significantly decreased(P<0.05). The expression of IL-4 was positively correlated with the urine protein of 24 hours(P<0.05), and had no significant correlation with the urine red blood cell count.(5) The relative expression of m RNA in rat splenic lymphocytes IL-4: the groups of A,B,C,Dand E are respectively0.1897±0.0010 、 0.8314±0.0013 、 0.5901±0.0008 、0.6617±0.0018、0.3822±0.0007.After treatment, C, D, E group urinary red blood cell count was decreased(P<0.05) compared with the B group, the E group decreased most significantly(P<0.05).(6) The expression of IL-4 protein in rat kidney: A,B,C, D and E are respectively0.137±0.004, 0.701±0.003, 0.390±0.001, 0.517±0.004, 0.289±0.002, After treatment, C,D, E group urinary red blood cell count was decreased(P<0.05) compared with the B group, the E group decreased most significantly(P<0.05).Conclusion:(1) 1,25(OH)2D3 may reduce the level of proteinuria in Ig A nephropathy rats, and play a role in renal protection.(2) 1,25(OH)2D3 may regulate the expression of Ig A in rats with IL-4 nephropathy and play a role in immune regulation.
