| Objective:To evaluate efficacy and safety of Jiahua tablet for diabetic kidney disease (DKD).And to compare Jiahua tablet,valsartan capsule as well as the effect of combined treatment differences.Methods:The method of randomized controlled was adopted.210 patients of phase â…¢, â…£ of DKD were randomly divided into 3 groups.The groups were as follows:Jiahua ta-blet group,valsartan group and combined group,70 cases in each group.Three groups we-re given low salt, low fat diet of diabetes mellitus.According to the individual blood s-ugar levels,appropriate hypoglycemic plan was set up;Blood pressure was high to antihy-p ertensives oral(not ARB or ACEI). Jiahua tablet group were treated with jiahua tablets 1. 8g,3 times per day,orally.Valsartan group were given valsartan capsule 80mg/d. Combined group were performed in both Jiahua tablets 1.8g 3 times per day, and valsartan capsul-e s 80mg/d.The duration of intervention was 12 weeks.Recording the general information of patients.ACR,UTP,SBP,DBP,TC, LDL-C,FBG,PBG,neutrophils,lymphocyte and other indi-c ators were measureed at 0,12weeks;Calculate NLR as well as conducting statistical analy-sis.Results:(1)phase â…¢ of DKD:The comprehensive effective rate of Jiahua tablet group, valsartan group and combined group was 82.86%,78.13% and 94.12%. The results showed that level of ACR in three groups was significantly decreased(P<0.01),and the differences between three groups were not obvious (P>0.05).(2)phase â…£ of DKD:The comprehensive effective rate of Jiahua tablet group, valsartan group and combined group was 81.82%,65.71%,90.63%.Jiahua tablet group and combined group resulted in a marked decrease in UTP level after treatment(P<0.01),which was superior to the valsartan group(P<0.05). (3)FBG, PBG, HbAlc of three groups were significantly decreased than before(P<0.05),while no differences was found between three groups(P>0.05).(4)Jiahua tablet group and combined group resulted in a marked decrease in TC, LDL-C level after treatment(P<0.05),which was not superior to the valsartan group(P>0.05).(5)The results showed that level of BUN, Scr of Jiahua tablet and combined groups were decreased(P<0.05),while ingcreasing in valsartan group.No significant differences were found between three groups(P>0.05).Along treatment process, no obvious adverse reactions appeared.(6)A positive correlation was found between NLR and ACR (r=0.633, P=0.000).phase â…¢ of DKD:N and NLR value were reduced after treatment in combined group(P<0.05),which were superior to other two groups without significantly diffrence.phase â…£ of DKD:N and NLR were found decreasing while L increasing in Jiahua tablet and combined groups(P<0.05),which were considered promising than valsartan group(P<0.05).Conclusion:(1)Jiahua tablet can obviously decrease urinary protein content,as well as lower the level of lipid in patients with DKD at the stage of â…¢,â…£.At the same time without obvious side effects, which prompts jiahua tablet has good security.(2)Combined treatment not only can obviously enhance the effect of reducing proteinuria, but also can combine superiority of jiahua tablet to improve blood lipid level with antihypertensive effect of valsartan capsule, which may contribute to strengthen the control of risk factors of DKD.(3)Jiahua tablet may protect kidney through improving kidney inflammatory state and other ways. |
