The Clinical Study Of Jiahua Tablet In The Prevention And Treatment Of Patients With Contrast Induced Nephropathy After Aoronary Angiography Or Interventional Therapy

OBJECTIVE To observe the effect of Jiahua tablet on renal injury index in patients with contrast-induced nephropathy after coronary angiography or interventional therapy,and to evaluate its clinical efficacy in the prevention and treatment of contrast-induced nephropathy,in order to provide a new clinical evidence for the diagnosis and treatment of contrast-induced nephropathy in Traditional Chinese Medicine(TCM).METHODS 80 patients with coronary angiograry or intreventional treatment were sel-ected and randomly divided into treatment group and control group with 40 cases each.Based on such routine treatments of control group as antihypertensive methods(calciuma-ntagonist,? receptor blocker,a receptor blocker,diurecits and the like),statins and aspirin,the treatment group was treated with 4 Jiah-ua tablets at a time,3 times pre day from preoperative 72h till postoperative 72h in a week.Such changes of index were observed as serum creatinine(Scr),serum urea nitrogen(BUN),glomerular filtration rate(eGFR),Cystatin C(Cys C),Urinary n-acetyl-?-d-glucosaminidase(NAG),urinary ?-galactosidase(GAL),high sensitive C-reactive protein(hsCRP),interleukin-6(IL-6),interleukin-8(IL-8),and the incidence rate of contrast-induced nephropathy were observed between the two groups.RESULTS 1.After postoperative 72h,the incidence rate of contrast-induced nephropathy in the treatment group was obviously lower than that in the control group(P<0.05).2.After postoperative 24h,the levels of Scr,BUN,Cys C,NAG,GAL,hsCRP,IL-6 and IL-8 in the two groups were significantly higher than those of preoperation,and the level of eGFR was markedly lower than that before operation(P<0.05,P<0.01).The levels of CysC,NAG,GAL,hsCRP,IL-6 and IL-8 in the treatment group was obviously lower than that of the the control group(P<0.05,P<0.01).3.Such the levels of the postoperative 72h as Scr,BUN,CysC,eGFR,hsCRP,NAG and GAL in the treatment group were basically the same as those before the operation(P>0.05),the postoperative 72h levels of Scr,BUN,CysC,NAG,hsCRP,IL-6 and IL-8 in the control group were significantly higher than those before the operation,while the level of eGFR in the control group was significantly lower than that before the operation(P<0.05,P<0.01).The levels of Scr,BUN,Cys C,NAG,GAL,hsCRP,IL-6 and IL-8 in the treatment group were obviously lower than that of the the control group,while the level of eGFR in the treatment group was obviously higher than that of the control group(P<0.05,P<0.01).CONCLUSION Jiahua tablet can significantly reduce the levels of SCR,BUN and Cys C,NAG,GAL,hsCRP,IL-6 and IL-8,increase eGFR,reduce the incidence rate of contrast-induced nephropathy,and play a better role in protecting renal function.
