Chronic subdural hematoma is a common disease in Department of Neurosurgery. For the patients with spontaneous intracranial hypotension accept neurological treatment more likely, the neurosurgeon is unfamiliar with the management of spontaneous intracranial hypotension in daily clinical practice. The clinical features of chronic subdural hematoma due to spontaneous intracranial hypotension which is with the presence of cerebrospinal fluid leakage frequently are unique, comparing with that of traumatic chronic subdural hematoma. The neurosurgeon should become more familiar with the special clinical characteristics of chronic subdural hematoma due to spontaneous intracranial hypotension.Objective:The survey to the clinical findings of chronic subdural hematoma due to spontaneous intracranial hypotension aim at putting a further understanding of chronic subdural hematoma, contributing to the diagnosis and treatment of related diseases and further study.The object of study:According to the inclusion criteria and exclusion criteria, cases enroll hospitalized in Sir Run Run Shaw Hospital affiliated to Zhejiang University during the period from June 2012 to June 2015, cases of chronic subdural hematoma due to spontaneous intracranial hypotension as intracranial hypotension group, cases of traumatic chronic subdural hematoma as traumatic group.Methods:The clinical data were collected from the intracranial hypotension group and the traumatic group separately, especially in the age, gender, imaging findings, treatment, efficacy and other aspects of information data. The Fisher exact test were used for statistical analysis. Combined with clinical experience and literature data, the interpretation of statistics results present.Results:Compared with the patients with traumatic chronic subdural hematoma, patients diagnosed with chronic subdural hematoma due to spontaneous intracranial hypotension more frequently found in young male adult at onset and with bilateral subdural hematomas more likely, need less surgical intervention, and have a higher ratio of secondary surgery. Patients benefit from an epidural blood patch with chronic subdural hematoma due to spontaneous intracranial hypotension. This study suggests that spontaneous intracranial hypotension as a cause of chronic subdural hematoma is easily overlooked by neurosurgeons. The research results should be further verified by large sample randomized controlled trials.
