ObjectiveTo study the clinical efficacy of Li-Shang-Xiao-Zhong oral liquid combined with low-molecular-weight heparin(LMWH) in the prophylaxis of Postoperative lower-limb DVT of the elderly with hip fracture. Evaluate its function in the Prophylaxis of DVT after hip fracture surgery, and provide reference for the clinical application of Traditional Chinese Medicine(TCM) in prophylaxis of Postoperative lower-limb DVT.MethodsAccording to the inclusion criteria, we selected 52 cases of elderly patients with hip fracture (femoral neck or intertrochanteric fracture) from orthopeadic trauma department of the First Affiliated Hospital of Guangzhou University of TCM, from August 2015 to February 2016. These cases were randomly divided into test group and control group, each group 26 cases. On the basis of conventional treatment, the test group received Li-Shang-Xiao-Zhong oral liquid combined with LMWH anticoagulant therapy, the control group received LMWH anticoagulant therapy alone. Then we observed two groups of patients with plasma D-dimmer, plasma homocysteine(Hcy), C-reactive protein(CRP), fibrino-gen (Fib) and the change of thigh circumference on the first day and seventh day after surgery. All the patients received vascular ultrasound examination on postoperative seventh day, and calculated the incidence of DVT. All data were analyzed by using statistical software Stata/SE11.0, and the difference was considered statistically significant (P<0.05).ResultsComparing the baseline data (gender, age, fracture site) of two groups of patients, there was no statistically significant difference between the two groups. By the analysis of two groups on postoperative first day limb swelling, plasma D-dimer, CRP, Fib and Hey, no statistically significant difference was found. There were 5 cases occurring DVT, and the total incidence was 9.62%. There were 2 cases of DVT in test group, with the incidence of 7.69%, while 3 cases of DVT in the control group, with the incidence of 11.54%. There was no significant difference between two groups(P=0.64). Making a statistical comparision of two groups about limb swelling (P=0.002), D-dimer(P=0.0198), CRP (P=0.011), Fib (P=0.036) on postoperative seventh day, the differences were statistically significant; Comparing the two groups about plasma Hey level on postoperative seventh day, the difference was not statistically significant (P=0.15).ConclusionsLi-Shang-Xiao-Zhong oral liquid combined with LMWH can effectively prevent the Postoperative lower-limb DVT of the elderly people with hip fracture, and its effect is equal to applying LMWH alone. However the combination treatment has certain advantages in these aspects such as improving hypercoagulable state, reducing postoperative inflammation and eliminating limb swelling, so it is worthy of clinical application and popularization.
